RN-On Call- PRN Nights

Providence is currently seeking a highly motivated RN who is interested in using his/her clinical skills in a Hospice care setting as a PRN On Call RN.
The On-Call RN is responsible for identifying and coordinating patient/family needs and for providing preventative and supportive care to the hospice patient and family. The On-Call RN utilizes assessment, intervention, and teaching skills to maximize patient comfort and to enhance the quality of life. The On-Call RN assumes on-call coverage for the designated teams and functions as an Interdisciplinary (IDT) team member to provide hospice care, support, and interventions after hours.
Responsibilities:
- Provides emergency, PRN, routine, and admission visits as necessary or scheduled.
- Assumes primary responsibility for hospice patients during on-call hours.
- Provides ongoing assessments of physical, psychological, and social and spiritual needs of the hospice patient and family and initiates appropriate interventions.
- Reviews clinical referral information and responds to requests and inquiries as appropriate Collaborates with Administrator On-Call regarding the eligibility and appropriateness of client for hospice services.
- Assesses and evaluates patient's status by following the plan of care, regularly reevaluating patient, and family/caregiver needs, participating in revising the plan of care as necessary based on patient status and needs.
- Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing interventions/actions. Includes the patient and the family in the planning process.
Qualifications:
- Hold a current unencumbered license as a Registered Nurse.
- Hospice/home health experience preferred but not required.
- Must be computer proficient in typing and various programs, including background in EMR.
- Possess and maintains current CPR certification.
- Must be flexible to adjust to changing tasks frequently.
- Must have a valid driver’s license, reliable vehicle, maintain valid auto liability insurance, and be able to drive throughout service area under a variety of weather conditions.
- Excellent verbal and written communication skills.
Job Type: Part-time
Pay: $100.00 - $200.00 per day
Expected hours: 8 – 48 per week
