My Diabetes Tutor empowers individuals with diabetes through virtual programs offering essential education and support. Our Diabetes Education Telehealth program, aligned with ADA standards, provides ten virtual sessions for comprehensive learning. Accredited by the American Association of Diabetes Educators (DEAP), we maintain the highest standards in delivering effective diabetes management.
Summary of Role
The Registered Dietitian (RD),Certified Diabetes Care and Education Specialist (CDCES) plays a pivotal role in delivering high-quality diabetes self-management education and support (DSMES) in accordance with national standards of care and practice guidelines. He/she will also provide medical nutrition therapy (MNT), assessing clients' dietary needs, evaluating medical nutrition conditions, offering nutritional education and interventions, and supporting clients in reaching their health goals, all done through telecommunication technologies.

- Minimum Weekly Availability: 16 hours (additional hours available if desired): hours specifically required between the hours of 5-9 pm PST on weekday evenings and/or weekend days

Job Duties and Responsibilities
- Conduct comprehensive assessments of individuals and their support systems to develop collaborative and personalized education plans.
- Deliver DSMES aligned with participants' goals, utilizing interactive methods and visually-engaging materials within the defined scope of practice.
- Provide Medical Nutrition Therapy when indicated.
- Facilitate self-care goal setting using the AADET framework.
- Serve as a participant advocate, ensuring their unique needs and concerns are addressed.
- Thoroughly document all DSMES sessions and track goal achievement throughout program completion.
- Foster effective communication and collaboration with referring providers and team members to enrich participant experiences and improve outcomes.
- Recognize when participants' needs extend beyond the scope of practice, necessitating referral to other healthcare providers and/or DCES.
- Support the Quality Coordinator in collecting participant outcomes, aligning with continuous quality program initiatives established by the Advisory Board.
- Identify areas for improvement in care delivery and participant experience.
- Actively participate in the Advisory Committee when invited, contributing valuable insights and expertise.
- Deliver evidence-based diabetes education while promoting participant engagement.
Qualifications and Skills
Qualifications:
- Fluent in Spanish
- Education and Credentials:
- Bachelor's degree in Dietetics, Nutrition, or a related field.
- Registered Dietitian (RD) credential from the Commission on Dietetic Registration.
- State licensure as required.
- CDCES
- Experience:
- Preferred 5 years of experience as a practicing dietitian
- Proven experience in providing nutrition counseling and education to diverse populations
- Specialized Knowledge:
- Diabetes Medications, Continuous Glucose Monitors
- Insulin Pump knowledge preferred
- Client Relationship Management:
- Demonstrated ability to establish and maintain positive client relationships.
- Proven track record of successful collaboration with healthcare professionals and interdisciplinary teams.
- Preferred: telehealth experience

Job Type: Part-time
Pay: $45.00 per hour
