NURSE MANAGER-PATIENT COUNSELOR JOB DESCRIPTION
JOB SUMMARY:
The Nurse Manager-Patient Counselor works with the Chief Clinical Operations Officer to provide counseling, education and options to patients confronted with unplanned pregnancies.
JOB DUTIES:
Provides patient assessment and evaluation as well as pregnancy, abstinence and STD education to patients. STI and Pregnancy point of care testing. Serves as the risk manager designee.
QUALIFICATIONS:
Education: Graduate of an accredited school of nursing and currently licensed in Florda. Must be BLS certified. Adheres to a consistent pro-life position and agrees with the organization’s statement of faith and mission.
JOB SKILLS:
· Assist patients to achieve the best state of health through their own actions
· Assesses and systematically collects data related to patient health status and works with the care team to coordinate and develop patient care plan
· Collaborates with team members to provide a smooth, organized and efficient flow of patient care
· Coordinates all clinic functions
· Reviews and revises policies and procedures
· Responsible for reviewing and approving employee hours and PTO
· Provides new staff orientation
· Conducts reviews with direct reports
· Assures equipment, supplies and staff are available
· Responds to patient crisis situations
· Collaborates with Patient and Volunteer Coordinator
· Carries out standing orders directed by the Medical Director
· Records patient histories, performs pregnancy and STI point of care testing
· Evaluates patient’s response to plan of care
· Assumes responsibility and accountability for effectively managing care of individual patients
· Completes accurate and timely documentation in Electronic health record
· Serves as mentor and resource to other team members
· May float to other clinics on “as needed” basis to accommodate operational needs
· Works independently within their scope of practice to achieve appropriate outcomes
· Maintains a level of professional development through continuing education and participating In-services, training and workshops
· Reflects understanding and strives to meet CPCI mission and standards of care
· Provides patient education and counseling
· Participates in staff training and in-services
· Completes annual HIPAA, biomedical waste tand Brightcourse training courses
