We are seeking a compassionate and dedicated Admission RN to join our interdisciplinary team to ensure that our patients receive the best possible care during their most critical times.
Job Summary:
The Admission RN/Clinical Liaison will be responsible for performing admissions and administering nursing care for patients of all ages in their place of residence. The role includes coordinating care with the interdisciplinary group/team, patients, families, and referring agencies. This position requires someone who can assume responsibility for the coordination of care effectively and compassionately.
Responsibilities:
- Assess hospice patients using Nursing Assessment and LCD criteria to identify physical, psychosocial, and environmental needs.
- Develop and document individualized care plans in coordination with the assessment.
- Provide nursing care while adhering to infection control measures.
- Collaborate with an interdisciplinary group/team to ensure comprehensive patient and family care.
- Communicate with attending and hospice physicians, and other medical professionals involved in the patient’s care.
- Ensure timely submission of all required paperwork.
- Participate in hospice-sponsored in-service programs and contribute to the Hospice’s Quality Assurance and Performance Improvement (QAPI) program.
- Promote the hospice philosophy and adhere to administrative policies.
Qualifications:
- Education: Graduate from an accredited nursing program (Diploma, Associate, or Bachelor’s degree).
- Licensure: Current State RN license and driver’s license.
- Experience: Minimum of two years as a Registered Nurse in a clinical care setting and one year in hospice care.
- Skills: Strong nursing skills, knowledge of state and federal regulations, excellent interpersonal skills.
- Transportation: Must have reliable transportation and valid auto liability insurance.
Working Conditions:
- This position involves working in patients' homes under various conditions and may include exposure to blood, bodily fluids, and infectious diseases.
- Ability to work a flexible schedule, travel locally, and respond to PRN emergency calls.
- Physical requirements include prolonged standing, walking, lifting up to 50lbs, and managing stressful conditions.

Pay: $65,000.00 - $80,000.00 per year
