POSITION SUMMARY: The Follow-up Specialist responds to incoming calls, follow up and provide care coordination services to designated callers and clients.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Respond to calls from clients, providers, educators, health professionals and social service agencies about health care services.
- Use telephone, email, and text platforms to link families with screening tools & provide results, referrals and learning activities.
- Follow up with screening reminders and care coordination to ensure that families access services.
- Help identify, document, and resolve service gaps and barriers.
- Handle document calls, collect required data and produce reports.
- Engage in collaborative problem-solving to identify and prioritize problems and potential solutions.
- Assist in setting appointments, providing reminders and follow-up with client.
- Other duties as assigned. QUALIFICATIONS & SKILLS:
- Minimum of 1 year of relevant experience and/or training, or equivalent combination of education and experience.
- Strong knowledge of administrative and clerical procedures.
- Proficient in computers and relevant software applications and practice management technology.
Possession of strong problem-solving skills and sound judgment.
Ability to
Communicate effectively both orally and in writing.
Demonstrate proficiency in MS Suite
Establish and maintain cooperative and effective working relations with others.
Learn, apply and explain policies, procedures, rules, and regulations.
Analyze situations accurately and determine an effective course of action.
Plan and organize work.

PREFERRED EXPERIENCE:
Prior medical billing and administrative experience in a private medical office.
Customer service.

WORK ENVIROMENT & PHYSICAL DEMANDS:
Ability to communicate clearly in person and over the telephone
Ability to read written material and computer screens.
Ability to enter data into a computerized record and to spend multiple hours on telephone & computer
Occasional lifting (up to 40 pounds)
