Schedule: Weekends Only (8am-5pm Saturday and Sunday) with every 6th weekend off
Position Overview:
Home Health Care Inc. is seeking a Home Health Weekends Only Intake Coordinator to join our team in Golden Valley! As an Intake Coordinator, you will be the initial point of contact for new patients and their families, playing a vital role in the seamless transition of patients into our care. Your responsibilities will include conducting initial screenings, verifying insurance coverage, coordinating with clinical staff to schedule patient visits, and maintaining accurate records.
You will work closely with patients, families, healthcare providers, and insurance companies to ensure that each patient receives the appropriate care tailored to their individual needs. Intake Coordinator Responsibilities:
Master the Art of Triage: Efficiently triage incoming phone calls, ensuring that each call is handled with the utmost care and urgency.

Hospital In-Basket Management: Skillfully manage hospital in-baskets, keeping all communications and records in perfect order.

Chart Creation: Craft comprehensive patient charts that provide a clear and accurate picture of each patient's health status.

Confirmation Calls: Make friendly and informative confirmation calls to patients and their families, ensuring they are well-prepared and informed.

Insurance Verification: Diligently verify insurance information, ensuring all services are covered and reimbursable.

Versatile Support: Perform a variety of other duties as assigned, showcasing your adaptability and commitment to exceptional service.
Intake Coordinator Qualifications:

Experience in a healthcare office, ideally in a home health setting preferred

Sharp, very customer service-oriented individual with the ability to multi-task will also be considered

Demonstrated effective verbal and written communication skills

Comfortable navigating and utilizing electronic medical records and computer programs

Maintains a positive outlook and approach to communication with clients, families, and other professionals

Excels at time management and prioritization of the most critical tasks

Upholds a professional and respectful attitude in all aspects of the role
