ORGANIZATION: Weston United Community Renewal Inc. is a not for profit organization located in Harlem, New York. For over 30 years, Weston United has provided housing and supportive services to participants with histories of chronic homelessness, mental illness, and substance abuse. Weston United employs over 100 employees who contribute to a rich and diverse work environment.
- Provide at least 2 face-to-face contacts with consumers in their apartments each week; ensure that consumers are living in safe, clean and decent environments; ensures consumers have adequate ADL skills by demonstrating, training and coaching; assist consumer with assessing needs and strengths to set goals and objectives; timely completion of required documentation for every client contact as per agency policy including but not limited to admission notes, progress notes, service plans and discharge follow up. Ensure documentation meets regulatory guidelines for level of care.
- Ensures that consumers are linked to all necessary ancillary services including psychiatric, medical and dental treatment, vocational and educational resources, and legal and financial entitlements; provide crisis intervention in acute episodes of emotional and physical distress in order to contain consumer’s regression and to ensure safety of consumers; completes follow-ups 30, 60 and 90 days documentation of discharged consumers.
- Represents the agency at conferences and meetings of referral sources, treatment providers and professional and community groups; ensures that consumers have appropriate seasonal clothing; monitor and evaluate whether or not consumers’ needs have been met by the ancillary services and make recommendations to supervisor; assist consumers with entitlements, budgeting and managing utilities.
- Escorts consumers to housing, benefits, and psychiatric/medical appointments when required.
Educational Requirements: Bachelors degree required. Masters degree preferred.
Qualifications and Experience: At least one years’ experience with Case management and carrying a designated case load. Experience working with adults who have a severe and persistent mental illness with or without a history of substance abuse preferred. Candidate must have good interpersonal skills; excellent written and verbal communication skills. Proficient in Microsoft Office applications including Word, Excel and knowledge of DHS CARES and AWARDS preferred. Experience working with mental health, crisis intervention and managing trauma triggered behavior. Excellent time management skills; stress resistant, flexible; team player; open feedback; creative; excellent organizational skills; willingness to preform CPR/First Aid/AED/CPI/Naloxone.
