A Brief Overview:
Disclosure: This is a temporary grant-funded position – should financial funding for this role be discontinued at any time or cease beyond the duration of the one-year grant period, the position will no longer remain active.
The Nutrition Care Coordinator (NCC) identifies and prioritizes nutritional interventions that support patients through transitions of care. The NCC advocates for patients with respect to nutrition support issues, participates in discharge planning, and provides direct patient support post-acute care discharge. The NCC supports organizational progress toward providing comprehensive, patient-centered, coordinated, and accessible nutrition care.
What you will do:

Research best practices for innovative ways to enhance programs to support access to nutrition related interventions across the transitions of care.

Provides care coordination to ease transition from acute care to home, with a focus on the following patient types: patients with diagnoses that require ongoing nutritional support, patients with food insecurity or identified food access needs, surgical patients, patients with diabetes, malnourished patients.

Conducts nutrition education when appropriate and as related to transitional care needs including, but not limited to, enteral or parenteral nutrition, hydration, ostomy care, modified diets and malnutrition care.

Assists patients and care team with nutrition-related logistics of discharge planning.

Coordinate referrals for outpatient nutrition education and other nutrition-related follow up care as appropriate.

Participates in and monitors ERAS protocols and assists patients with nutrition-related transition to home.

Provides ongoing nutrition-related support via telephone, electronic communications, and/or home visits as needed.

Drives planning and implementation of data collection and outcomes research as related to the job function.

Electronically documents all aspects of nutrition care.

Attends special meetings and rounds as assigned.

Assists with quality improvement audits and projects.
-
Participates in interdisciplinary committees.
-
Participates in continuing education programs and assists with in-service training programs.
-
Collaborate with other hospital services to ensure seamless services, and create/maintain initiative-related protocols.

Provide training to staff as needed related to position.
-
Provide quantitative reporting on monthly and quarterly progress.
-
Independently anticipates and always responds appropriately to the age related patients’ psychosocial and physical needs.
What you will need:

Bachelors Degree Required Or
-
Masters Degree Preferred
-
Ability to communicate effectively with interdisciplinary medical team

Computer skills

Licensed Dietitian Nutritionist (LDN) - Illinois Department of Financial and Professional Regulation (IDFPR) Required And

Registered Dietician (RD) - Commission on Dietetics Registration (CDR) Required
