Responsibilities:
· Assists with monitoring compliance with agency policies and procedures related to clinical services. Completes audits, conducts reviews and assists with quality assurance duties as assigned. Is alert to potential areas of concern or risk, and actively seeks ways to monitor compliance and address issues early.
· Participates in and provides leadership for the staff for regular interdisciplinary team meetings; assists in the development, review and revision of the patient/family plan of care; facilitates problem solving sessions; and delivery of team support.
· Oversees the daily clinical supervision of clinical personnel and assists with orientation of new staff.
· Assists with the planning, coordination and delivery of the interdisciplinary team interventions as established in the plan of care and in accordance with physician orders.
· Assigns patients/families to appropriate staff member(s).Monitors assignments to ensure appropriate match and assist with any issues or concerns. Ensures visits are made per the plan of care and physician orders.
· May assume regular case management duties when required.
· Participates in on-call duties.
· Provides professional nursing care by utilizing all elements of nursing process in order to meet the individualized needs of each patient.
· As needed, completes initial or comprehensive assessments of patients and caregivers/families to determine immediate needs. Completes physical assessments and histories of current and previous illness(es).
· As needed, creates and implements the initial and comprehensive individualized plan of care based on assessment(s). Collaborates with the interdisciplinary team. Initiates the plan of care, obtains needed physician orders, and makes necessary revisions as patient status and needs change.
· As needed, reassesses and reevaluates patient's status on a continuous basis. Collaborates, updates, and carries out the plan of care. Regularly re-evaluates the patient and family/caregiver needs. Revises the plan of care as necessary and obtains new orders as required. Maintains communication with the attending physician.
· Develops or assists staff to develop care plans that reflect individualized goals, based on nursing diagnosis and incorporates palliative nursing actions.
· Initiates or assists staff to initiate appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care.
· Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required.
· Conducts education/admission visits with caregivers and patients which includes providing information about available services, as well as listening to the individual needs and responding appropriately.
Qualifications:
· Graduate of an accredited school of nursing
· Licensed to practice as a RN in the State of South Carolina with a license in good standing
· Maintain a current CPR certification and valid SC driver's license.
· Minimum of two (2) years of nursing experience required.
· One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care preferred.
· Bachelor's degree, with one year of hospice care experience preferred.
· Skilled in administrative performance, including expertise in both written and oral communication, supervision of staff, documentation requirements, service coordination and case management.
· Ability to execute an effective, supportive, sensitive approach in response to needs of staff and patient/family.
· Detailed oriented and strong problem solving skills
