As an Intake Coordinator, you'll play a crucial role in ensuring a continuous flow of potential patients into all our clinic locations. You'll work closely with internal and external departments, providing vital support. We seek a self-motivated individual with a strong work ethic and a “can-do” attitude.

Key Responsibilities:
- Organize and coordinate patient care, services, and visits to ensure seamless transitions and continuity of care.
- Conduct phone interviews with patients to gather essential information, confirm demographics, assess social support systems, schedule initial visits, and review medications and insurance coverage.
- Evaluate patients' needs by reviewing medical records and pertinent documents.
- Collaborate closely with nurses, NPs, MDs, and other healthcare team members to coordinate patient care.
- Provide clear explanations of available services to patients, families, and caregivers.
- Schedule home visits with patients to assess progress and adjust care plans as necessary.
- Review patients' insurance coverage to ascertain eligibility for home health care services.
- Maintain professional relationships with caregivers, medical staff, patients, and their families.
- Perform billing tasks as directed by the Supervisor.
- Maintain and report various administrative records.
- Establish and nurture relationships with neighboring medical agencies for referrals.
- Conduct all activities with utmost professionalism and confidentiality.
- Identify work-related issues and propose solutions.
- Participate in continued education and staff in-service trainings.
- Ensure compliance with Medicare and Medicaid regulations.
- Adhere to infection control policies.
- Support quality improvement programs and initiatives.
- Undertake additional duties as required by the Supervisor.
Qualifications:
- Previous experience in clerical roles.
- Familiarity with medical software and equipment.
- Strong rapport-building skills with patients and facility staff.
- Proficient problem-solving and critical thinking abilities.
- Excellent communication skills, both verbal and written.
- Competency in Microsoft Word and Excel.
- Positive demeanor and ability to relate well with patients and staff.
- Exceptional organization and multitasking skills, with the capacity to work independently.
- Proficiency in written documentation and computer competency.
Work Environment:
- Remote in-home and occasional in-clinic setting.
Education and Experience:
- Associate degree (Preferred).
- Previous experience in Home Health services or Hospice environments (Preferred).
- Ability to set objectives, evaluate staff competencies, assign tasks, foster team alignment, and cultivate a motivating and safe working environment.
- Proficiency in enterprise applications with intermediate knowledge of Microsoft Office.
- Ability to build trusted relationships and networks, leveraging personal connections across various supervisory and leadership levels.
