Position Overview:
Caregiving has increased in the recent years and is projected to continue to rise in the near future. Whether they became a caregiver gradually or suddenly due to a crisis, caregivers themselves often do not have adequate support to navigate the obstacles they face. As the Care Manager for the Family Caregiver Support Program, this individual will work directly with caregivers to determine their needs. Providing counseling, education, and relief, the Care Manager can empower caregivers enabling them to better care for themselves and those they support.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Conduct intake and assessment with clients to determine eligibility, need, and fit for services.
- Develop individual care plans in collaboration with clients based on their specific needs.
- Provide short-term counseling to clients.
- Review and evaluate client progress with ongoing and timely case management.
- Organize and facilitate support groups, trainings, and education-based outreach events.
- Document units of service and client data accurately for funding and future analytics.
- Maintain confidential records and reports.
Job Qualifications:
- Bilingual in English and Spanish required.
- Experience working directly with older adults and/or caregivers from diverse communities.
- Bachelor’s degree in social services-related field; Degree in social work preferred.
- Proficient in the use of personal computers and related software, including internet, e-mail, point-and click reporting programs, and Microsoft Office.
- Possess a valid class C driver’s license; experienced driver with a clean driving record.
- Demonstrated ability to work both independently and as an active team member.
- Completion of Adult First Aid | CPR AED training or completion within 30 days after hire.
Supervisory Responsibility
This position has no direct supervisory responsibilities.
Work Environment
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

The employee is often required to sit and use their hands and fingers, to handle or feel and to manipulate keys on a keyboard. The employee is occasionally required to stand, walk, reach with arms and hands, climb or balance, and to stoop, kneel, crouch or crawl. Vision abilities required by this job include close vision. This is largely a sedentary role; however, some filing is required. This would require the ability to lift 10 lbs., lift files, open filing cabinets and bend or stand on a stool as necessary.

Travel
Occasional travel to client homes is expected for this position. (company vehicle available for use)
Required Education and Experience
Bachelor’s degree in social sciences preferred; or four years related experience and/or training; or equivalent combination of education and experience.
Language Skills:
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, procedure manuals, ability to write routine reports and correspondence.
- Bilingual required; Spanish language fluency.
