Job Description
The Pharmacists, whether onsite or remote, will ensure safe and effective medication use, optimize therapy outcomes, and promote patient education and adherence.
ROLES and RESPONSIBILITIES
· Review written medication orders and prescriptions as well as those submitted electronically or through telehealth platforms to ensure accuracy, appropriateness, and compliance with regulatory requirements.
· Provide medication therapy management (MTM) services, including drug information, clinical assessments, and recommendations to optimize therapy outcomes and minimize adverse drug reactions.
· Conduct virtual medication counseling sessions with patients to discuss medication instructions, potential side effects, drug interactions, and adherence strategies.
· Collaborate with healthcare providers to reconcile patients' medication histories, identify discrepancies, and develop medication reconciliation plans to promote medication safety and continuity of care.
· Perform drug utilization reviews (DUR) to identify potential medication errors, duplications, contraindications, and therapeutic interventions and communicate findings to prescribers and patients.
· Process medication orders, dispense prescriptions, and coordinate medication delivery or pickup through mail-order pharmacies, automated dispensing systems, or e-prescribing platforms.
· Monitor patients' medication adherence through remote medication management tools, patient-reported outcomes, and assessments, and intervene to address adherence barriers.
· Ensure compliance with pharmacy laws, regulations, and standards of practice, including remote dispensing regulations, patient privacy laws (e.g., HIPAA), and medication safety guidelines.
· Provide tele-pharmacy services to patients and healthcare providers, including medication counseling, refill authorizations, prior authorization assistance, and medication therapy management (MTM) reviews.
· Maintain accurate and up-to-date records of medication orders, interventions, communications, and clinical assessments in electronic health records (EHR) or pharmacy information systems.
QUALIFICATIONS and REQUIRED SKILLS:
· Minimum of 5 years of experience as a licensed pharmacist, including experience in tele-pharmacy or remote pharmacy practice settings.
· Current and unrestricted pharmacist licensure in the state(s) where you intend to practice pharmacy, with eligibility for licensure in multiple states if providing services across state lines.
· Certification or training in remote pharmacy practice, tele-pharmacy, or telemedicine platforms.
· Previous experience in clinical pharmacy practice, medication therapy management (MTM), ambulatory care, or telehealth is beneficial.
· Strong understanding of pharmacology, pharmacotherapy, and clinical guidelines to assess medication appropriateness and optimize therapy outcomes.
· Excellent verbal and written communication skills to interact remotely with patients, healthcare providers, and interdisciplinary teams.
· Comfort using telemedicine platforms, electronic health records (EHR), pharmacy software systems, and communication tools for remote consultations and medication management.
· Ability to analyze medication-related problems, identify solutions, and make evidence-based recommendations to improve patient care remotely.
· Empathy, compassion, and patient-centered approach to counseling patients, addressing their concerns, and promoting medication adherence remotely.
· Keen attention to detail and accuracy in medication review, documentation, and communication to ensure patient safety and regulatory compliance.
· Effective time management and organizational skills to prioritize tasks, manage workflow, and meet deadlines in a remote pharmacy setting.
EDUCATION
· Doctor of Pharmacy (PharmD) degree from an accredited pharmacy school.
