JOB SUMMARY: Oversees the medical care of residential consumers including medical follow-up, medication, adult mental health rehabilitative services and nutrition. Provides education for both the consumers and the residential staff.
JOB RESPONSIBILITIES:
1. Demonstrates sound knowledge of psychiatric nursing standards of practice.
2. Performs initial nursing assessment and reassessment of clients according to applicable program standards and identified tools.
3. Identifies actual and potential health problems, analyzes the information gathered, and formulates nursing diagnosis, making timely and appropriate referrals and follow up for medical needs.
4. Identifies emergent medical problems and intervenes appropriately.
5. Utilizes psychotherapeutic interventions to assist clients in enhancing or improving the ability to manage life stressors.
6. Applies principles of medication administration and storage, demonstrating safe practice and accuracy.
7. Provides ongoing psycho education to clients and families especially in Re: to medication management, symptom management, and health education and documents same.
8. Proper administration of injectible psychiatric medication as prescribed by an M.D. on an emergency basis.
9. Educates consumers in the maintenance of optimal nutrition.
10. Provides medication education for both the client and staff.
11. Provides training to residential staff on policy and procedure of medication use, personal care services, and nutrition.
12. Inputs Adult Mental Health Rehabilitation Services and other documentation.
13. Provides nursing coverage in other programs on an emergency basis
14. Participates in treatment planning with clients, inter-disciplinary team members both within and outside the agency.
15. Provides documentation, including treatment plans and progress notes, according to agency standards.
16. Provides crisis prevention and intervention as needed.
17. Provides transportation of clients to their medical appointments.
18. Participates in staff meetings, trainings, in-services, and supervisory sessions.
19. Provides advocacy and education regarding self-advocacy for clients.
20. Assists clients with integration into the community concerning medical issues.
22. Interfaces with other provider agencies, medical providers, and families as needed.
22. Responsible for coordinating the medical care of all residential consumers.
23. Available for on-call coverage via pager for residences.
24. Adheres to requirements in General Skills Evaluation.
25.Will perform other duties and responsibilities as requested by VHS management without giving up the right to the grievance process.
26. Acts as liaison between pharmacists and residence.
EDUCATION: A license as a registered nurse in the State of New Jersey
EXPERIENCE/KNOWLEDGE: Two years related work experience in a mental health setting working with the mentally ill. Knowledge of basic computer skills.
JOB QUALIFICATIONS:
OTHER: A current New Jersey Registered Nurse license.
Computer literacy
Physical and auditory acuity sufficient to evaluate, treat, and monitor client needs, recognize clients, and maintain accurate records. Fine motor skills for legible written documentation and use of keyboard. Ability to work required hours and fulfill job requirements. Maintains CPR Certifications. Willingness to learn basic computer and word processing skills.
