SUMMARY:
The Primary Care Physician is directly responsible for performing and recording patient health history, patient examination and diagnosis, recommendations of medical treatment, rendering of on-site treatment and hospitalization services as necessary. The physician is also responsible for appropriate documentation in the patient chart.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Manages patient medical care including diagnosis, treatment, and follow-up
- Maintains appropriate documentation on patients clinical records
- Participates in Alivio Medical Center activities and clinical committees as assigned
- Maintains weekly office hours as negotiated in Principles of Practice including evening and Saturday hours, as required
- Participates in continuing medical education to meet standards of retaining license and Board Certification
- Participates in team conferences, health education, staff meetings, and committees as needed
- Acts as a Collaborating Physician for any Midlevel provider; including but not limited to Nurse Practitioners and /or Physician assistants
- Performs other related duties as assigned
This position requires compliance with all of Alivio’s written standards, including its

SUPERVISORY RESPONSIBILITIES:
Manages and supervises staff listed below, including hiring, training, evaluating and managing performance to meet departmental efficiency and performance measures in keeping with the Union Collective Bargaining Agreement (CBA), Personnel Policies and Procedures and prevailing employment laws as appropriate.

QUALIFICATIONS:
Education:
- Graduate of Accredited medical school with degree of Doctor of Medicine or Doctor of Osteopathy. Completion of Accredited residency program.
Certification or Licensure:
- Board certification in specialty or board-eligible (within the allowable period of time post-graduation). Unrestricted license to practice within Illinois. Current DEA certificate.
- Experience:
- Three to five years experience in a community health center. Experience working with underserved populations, preferred.
