General Description of the position:
· Supervises care partners
· Provides leadership and direction in the activities of daily living for residents.
· Provides direct care to the residents of the community.
· Encourages resident to function at their highest level of independence.
· Conducts initial screening interviews, trains, and supervises other Care Partners.
· Assists with scheduling.
· Acts as a liaison between Care Partners and Director of Nursing/RCC.
· Responds to resident medical emergencies
· Ability to communicate effectively with staff, resident responsible parties and outside service providers

· Maintains a working knowledge of all policies and procedures pertaining to the position.
· Maintains a working knowledge of the Care Partner job description.
· Effective communication with the care partner team.
· Generate enthusiasm and a spirit of cooperation.
· Establish standards of performance.
· Give guidance and follow-up to care partner staff needs.
· Take corrective action when deviations occur with care partner staff.
· Facilitate purpose and direction by setting goals and objectives with care partner staff.
· Develop a climate of trust, respect, and fair treatment.
· Facilitate and encourage creativity and vision by care partner staff.
· Provide performance evaluation feedback of care partner staff to the RCC.
· Be prepared to provide Health Services staffing coverage in the event of staff shortages or outages.
· Fill out incident reports as necessary.
· Read communication log and communicate with health services staff daily.
· Perform quality assurance checks.
· Monitor status of resident’s health condition and initiate actions in response to changing needs. Provide staff training for establishing proper plan of action and documentation when injury occurs.
· Oversee and ensure completion of state-required documentation and forms.
· Monitors infection control programs, specifically recognizing patterns of nosocomial (in-house) infections and their potential to spread.
· Other duties as assigned
Qualifications:
· Must have Caregiver experience
· A genuine concern for the elderly
· Must have excellent communication skills
· Must be able to work well as a team member
· Must be very dependable and reliable
· Must be able to take initiative in problem solving
· Ability to lead and motivate people
· Capable of making mature judgments
· Remain drug free
· Well groomed
· Ability to manage and prioritize a large workload
· Ability to maintain confidential information
· Must be CPR Certified
· Must have First Aid Training Certificate
· Must have abdominal thrust training
Working conditions:
· Sitting/Standing: up to 8 hours
· Lifting requirements: up to 200 lbs. with assistance.
Equipment used:
· Office equipment including but not limited to: telephones, computers, copiers and printers
· Occasional tasks may require the use of (but not limited to) lifts, needles and driving company vehicles.
Education:
High school diploma minimal, prior industry experience.
