Position Overview
The Reentry Case Manager is responsible for identifying needs, barriers, and appropriate interventions for program participants as they return home. This role will work with a team providing holistic, wraparound supportive services and resources, and will support participants in the advancement of their development, goals, and well-being. The Reentry Case Manager will work closely with program participants to assist in planning before, during, and after the program. This position reports directly to the Director of Reentry and is based in Los Angeles, CA.
Responsibilities and Duties
The essential functions of this role include, but are not limited to:
Oversee program enrollment, intake process, and program exit for program participants:
Conduct program orientation and intake process for accepted program applicants, including ensuring completion of intake forms and performing appropriate assessments.
Develop initial plans to stabilize participants and establish necessary vital accounts and documents, including individual development plans to address barriers, support asset development, and set goals.
Coordinate program enrollment logistics for participants, including but not limited to initiating and tracking parole transfers, assisting with gathering necessary documents (e.g., ID), and support establishing necessary accounts.
Conduct needs assessments and develop reentry plans to encourage goal setting that supports fellows developing into their best selves.
Lead onboarding/offboarding process for program participants during their move-in and move-out from campus, including reviewing residential agreements, house rules, campus policies.
Lead program exit development plan and implementation with participants leaving the program
Coordinate ongoing case management for participants during the program:
Administer relevant risk needs strengths assessments and develop comprehensive case plans to implement through course of program
Manage participant caseload to provide stabilization services and promote employment and housing retention
Develop monthly progress reports to demonstrate participant progress and challenges
Coordinate all participant schedules including approval of travel, overnight, and weekend passes, coordination of participant work schedules, and notification of changes to employment
Provide counseling and encouragement to pursue employment pathways that align with program career tracks
Provide ongoing support for program participants during program to ensure reentry and supportive service needs are met
Lead team case conferencing meetings and ensure care coordination across departments
Ensure accepted program participants maintain compliance with all applicable parole requirements, rules, and guidelines, including but not limited to understanding parole requirements and facilitating or coordinating parole required classes for participants
Conduct data entry, evaluation, and reporting:
Complete the timely and accurate submission of progress notes and/or records for all participant-related activities and track participant outcomes in database
Prepare and document intake interviews, intake documents, discharge plans, continuing support plans, etc.
Compile necessary departmental and funder reports and evaluations as appropriate
Develop and maintain database of relevant community resources
Support with additional organizational duties:
Coordinate trainings with staff and strategic partners
Uphold and promote the organizational mission, vision, culture, and goals
Other duties as assigned
Preferred Qualifications
3+ years of experience or education commensurate with a demonstrated and thorough knowledge of case management and reentry service delivery.
Motivational interviewing skills (focused on engaging justice-involved candidates).
Excellent people skills and ability to collaborate across different program divisions.
Proficiency with technology, and using various information management systems such as databases, Google or Microsoft Office products, etc. Experience with Salesforce is a plus.
Knowledge of evidence-based therapeutic techniques and trauma-informed care.
Excellent communication, active listening, and interpersonal skills.
Ability to work effectively in a team-based environment and collaborate with diverse stakeholders.
Cultural sensitivity and the ability to work with individuals from various backgrounds, especially justice-involved individuals
Knowledge of applicable city, state, and federal guidelines, and laws affecting mental health treatment services and reporting procedures.
Proficiency in maintaining accurate and confidential client records.
Salary and Benefits
This is a full-time, non-exempt position. The salary range for this position is $74,000 to $81,000. Full-time employees will be eligible for all benefits including vacation, sick days, and organizational holidays. You may participate in all benefit programs that CROP establishes and makes available to eligible employees, under (and subject to all provisions of) the plan documents that govern those programs. Work Schedule and Location
This role will be based out of our office located at: 2365 Pacific Avenue, Long Beach, CA 90806. The hours required for this role are 10:30 am - 7:00 pm Monday - Friday.
