QUALIFICATIONS:
- Minimum 2 years of clinical hospice experience (or 1 year hospice clinical plus 1 year hospice quality).
- Nursing degree required (LVN)
- Strong understanding of hospice regulations, compliance standards, and documentation requirements.
- Excellent attention to detail and exceptional organizational skills.
- Proficiency in using electronic medical records (EMR) systems.
- Effective communication skills to liaise with staff.
- Strong problem-solving and critical-thinking abilities.
- Demonstrated ability to work independently and collaboratively as part of a team.
- Excellent time management skills and ability to prioritize tasks effectively.
- As a Hospice Quality Nurse, you will play a pivotal role in ensuring the quality and compliance of our hospice services through meticulous review and documentation.
- Conduct comprehensive reviews of medical records to ensure compliance with documentation standards.
- Prepare charts for bi-weekly Teams IDG meetings, ensuring accuracy and completeness.
- Review all admissions and recertifications for eligibility, accurate diagnoses, complete medication profiles, comprehensive flowsheets, well-developed care plans, and other relevant orders.
- Evaluate visit notes, communication notes, and uploaded documents, such as history and physicals.
- Prepare documentation for Teams and physician signatures.
- Determine the relevance of diagnoses and medications for EOB addendums.
- Initiate Plan of Care documents and collaborate with staff to address any necessary corrections.
- Enter, troubleshoot, and schedule discipline orders for RNs, Chaplains, MSWs, and Hospice Aides.
Clinical Nursing and Informatics Education:
- Provide thorough nursing orientation for newly hired RNs and LVNs.
- Ensure staff's understanding of the components and requirements of compliant documentation.
- Familiarize staff with clinical tools and scores for effective hospice documentation.
- Maintain a comprehensive database of educational materials and update them as needed.
- Lead or actively participate in Documentation Compliance In-Services.
- Provide ongoing education, both one-on-one and in small groups, covering EMR usage and clinical practices.
- Attend Stand-Up meetings, survey preparation, and other staff meetings to share compliance reminders, EMR updates, and other relevant information.
QAPI and PIP Coordination and Implementation:
- Perform monthly and quarterly audits on clinical records, bereavement processes, falls, infections, incidents, adverse events, and complaints.
- Design, implement, and manage Performance Improvement Plans (PIPs) to enhance overall quality.
- Create comprehensive documentation of the PIP and audit all charts related to the plan.
- Present audit findings at quarterly QAPI meetings, contributing to ongoing improvement efforts.
- Collaborate with HPS consultants regarding workbook maintenance and compliance inquiries.
- Attend weekly meetings with upper management to provide updates on quality assurance initiatives.
- Participate in weekly or more frequent meetings with Clinical Supervisors to ensure effective communication and coordination.
