RESPONSIBLITIES:
1. Provide comprehensive support through physical health and behavioral health
interventions to adult individuals designated with a Serious Mental Illness
2. Assesses clients in accordance with established policies and procedures and clinical
protocols.
3. Develops treatment plans for clients based on assessment and diagnostic process
4. Coordinates client care or reports on interventions by meeting with client’s care team to
review efficacy of interventions and to revise plans of service for members
5. Develops and maintains appropriate level of relationships with clients and
formal/informal support systems
6. Provides support and information, education or guidance to clients and support system
concerning issues of treatment planning, advocacy, and medication management.
7. Monitors mental status of clients and response to treatment plans through face-to-face
contact/tele-health, or telephonic and updates treatment plans as needed
8. Notifies direct supervisor of actual or potential challenges with client, i.e., substance
abuse challenges, DTO/DTS, or significant health concern and crisis
9. Provides face-to-face and telephone crisis intervention
10. Completes safe/support plans with clients
11. Conducts groups as needed.
12. Transitions client to appropriate level of care upon conclusion of treatment
13. Provides information to clients and client’s support system concerning community
resources and linkages to other services.
14. Assists clients in securing appointments with support services and agencies, as needed.
15. Arranges for transportation for clients to community services as needed.
16. Maintains accurate, thorough and current documentation of contact with clients,
treatment progress and services received/provided.
17. Some traveling will be required for linkage to community services
18. To ensure compliance & adequate services, additional job duties may be required to
meet the needs of the program and or department.
19. Performs other duties, tasks, or services as assigned by Clinical manager or direct
manager
Job Type: Full-time
Pay: $28.00 - $33.00 per hour
