In our Licensed Practical Nurse role, you will be assisting in delivering nursing services to residents, charting in our P.C.C. system, and ultimately collaborating with other healthcare providers to serve our residents best.
Requirements:
- Registered Nurse
- Comfortable working independently with the population when needed.
- Compliance with MAT (Medication-Assisted Treatment) program protocols.
- COVID –19 Vaccination approved exemption status
Responsibilities:
- Hire, train, supervise, motivate, and evaluate caregiver staff to ensure residents' highest quality of care.
- Assist and Maintain staff schedule.
- Work directly with the Executive Director to evaluate residents and update care plans.
- Monitor the day-to-day work provided by the med techs, supervising, critiquing, suggesting, and educating the team on proper work standards.
- Coordinate the flow of information in the medication room in conjunction with the E.D. to develop procedures that assist in eliminating the opportunity for medication errors to occur.
- Coordinate the medication pass to the residents, supervising the medication aide and providing education, as necessary.
- Complete initial paperwork on all new admissions and routinely audit resident charts to ensure paperwork is complete and filed correctly.
- Coordinate the resident's move-in assessment and orients new residents to the facility.
- Facilitate and coordinate the discharge process for all residents discharged from the Community, including assisting the resident, family, and other healthcare providers to make this transition as easy as possible.
- If designated by D.H.S., complete each resident's initial assessment and service plan.
- Monitor and implement the service plan and assessment process, including:
- Updating service plans on a routine and timely basis, scheduling service plan meetings with residents, employees, and families
- Facilitating the service plan meeting to ensure each participant covers all information
- Routinely monitoring the process to develop new and better ideas for delivering care to the residents.
- Assist Community Relations in marketing efforts as the Executive Director assigns.
- Monitor care delivery during all shifts by scheduling routine monthly visits to the late evening and night shift caregivers.
- Assists and helps facilitate the monthly Community staff meetings.
- Organizes and assists with developing meetings with the resident council.
- Monitor Alert Charting
- Audits weekly medication rooms to ensure they are clean and compliant with our state policies.
- Reviews and document the care plan and chart notes as needed.
Review daily during a shift:
- Alert charting
- Home Health notes
- New orders
- M.A.R. R.C.C. to notify D.H.S. of any changes to the care plan.
- Monitor and coordinate the bathing and laundry.
- Perform the duties of a medication aide or caregiver in extreme staffing shortages.
Job Type: Full-time
Pay: $100,000.00 - $125,000.00 per year
