JOB TITLE: Case Manager
REPORTS TO: Program Manager
DEPARTMENT: Mixed Use Affordable Housing
CLASSIFICATION: Non-Exempt
HOURS: 35 hours per week
QUALIFICATIONS:
BA in Human Services or equivalent. Experience working with people with disabilities or older adults a plus. Clean, valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
JOB FUNCTION:
Provide case management services, housing support and assistance to individual’s age 55+, who have chronic health conditions and need assistance with activities of daily living. Assist residents with overcoming barriers associated with their health conditions so they may live in on-site in their apartments successfully. Provide information, training, hands on assistance and skill building to individuals served.
Job Type: Full-time
Pay: $40,000.00 per year
