Job Title: 11-7 Nurse Supervisor

SUMMARY Responsible for directing activities of nursing staff in Gwynedd House and Abington House by performing the following duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Supervises the delivery of care consistent with residents’ needs, the plan of care, the health services philosophy, policies and procedures, and standards of nursing practice.
Serves as consultant, educator, and role model to nursing staff in all matters of nursing practice.
Demonstrates knowledge of the nursing process in assessing resident status, in planning and executing care, and in evaluating the interventions provided.
Perform job duties of a team leader as necessary to include; evaluating residents, resident admissions and treatment’s according to acceptable nursing practice.
Administers medications to residents in accordance with acceptable standards of practice, demonstrating competency in knowledge of medications, effects, and therapeutic dose as needed.
Provide education and support to residents and family regarding the residents’ health status.
Makes apartment visits or delegates same as necessary to evaluate community member needs and to provide appropriate interventions.
Ensures timely documentation of resident care.
Coordinates transfer of resident to hospital, assisted living, or skilled nursing facility, transferring appropriate medical record information needed to ensure continuity of care.
Responds promptly to resident emergencies, administering first aid, cardiopulmonary resuscitation or other interventions as warranted by the situation.
Recognizes and promptly reports changes in resident condition, resident grievances or complaints, accidents, and unusual incidents to the Director of Nursing and/or Director of Health Services.
Demonstrates knowledge of fire safety procedures and provides direction to staff during alarms.
Coordinates and adjusts staffing patterns for 11PM – 7AM shift in accordance with total resident needs and state requirements.
Assists in replacing staff callouts as appropriate.
Ensures the completion of all event reporting on the 11-7 AM shift.
Ensures needs of 7-3 shift are met (i.e. staffing, supplies, report etc.) prior to leaving.
Assures prompt response to fire, off-hook, Igeacare emergency alarms in collaboration with the maintenance staff.
Recognizes and maintains appropriate infection control practices including Pennsylvania Act 52 requirements as assigned by the DON.
Evaluates employees who have sustained injuries while on duty and refers to the appropriate facility when needed.
Participates in orientation of new staff to Gwynedd House and Abington House.
Identifies areas for staff education and assist in providing staff education in-services.
Participates in quality improvement activities and data collection activities.
Assumes responsibility for maintaining certification in cardiopulmonary resuscitation and ongoing professional development through internal and external continuing education programs.
Maintains confidentiality of residents’ personal and medical record information.
Counsels and disciplines assigned staff as needed and participates in staffs’ annual performance evaluation.
Promotes clean, safe and neat environment for residents, staff and self.
SUPERVISORY RESPONSIBILITIES
Manages team leaders who supervise employees in Abington House and Gwynedd House.
Is responsible for the overall direction, coordination, and evaluation of nursing staff on 11-7AM shift. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include training employees, planning, assigning and directing work, and appraising performance. Supervising responsibilities of counseling, rewarding and counseling employees, addressing complaints and resolving problems. Works independently while communicating with the DON and/or the Director of Health Services as necessary during the 11-7AM shift.
QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE Graduate of an approved school of professional nursing, currently licensed in Pennsylvania as a Registered Nurse. Current CPR and intravenous therapy certification. Minimum of 3 year clinical experience in a long term care setting and prior supervisory experience required.
LANGUAGE SKILLS Ability to read and interpret documents such as physicians' orders and resident’s charts. Ability to clearly document appropriate information in resident charts. Ability to effectively present information and respond to questions from residents, family members, physicians and staff members.

Capability for prolonged standing and for walking long corridors many times per 8-hour shift; for frequent stooping and bending, and for lifting, pushing, pulling, and moving a minimum of 50 pounds when providing resident care and transfers. Ability to see and hear, or to use prosthetics that will enable these senses to function adequately to assure that position requirements can be fully met. Ability to read, write, speak and understand English. Sufficient manual dexterity to sustain prolonged periods of handwriting, to operate a computer, to operate a fire extinguisher, and to manipulate medical equipment, including vital sign measuring equipment, syringes, and medication containers.
