As a Primary Therapist/Social Worker, you'll play a pivotal role in enhancing the lives of our residents by providing compassionate therapy and social work services tailored to their unique needs.
Key Responsibilities:
- Individualized Therapy: Conduct comprehensive assessments and develop individualized therapy plans for residents, incorporating evidence-based practices and innovative therapeutic techniques.
- Group Therapy: Facilitate engaging and therapeutic group sessions that promote social interaction, emotional support, and cognitive stimulation among residents.
- Family Collaboration: Collaborate with families and caregivers to create a supportive environment that fosters understanding and effective communication about residents' needs and progress.
- Case Management: Manage a caseload of residents, ensuring continuity of care and proactive intervention to address behavioral, emotional, and social challenges.
- Crisis Intervention: Provide crisis intervention and support during times of emotional distress or behavioral escalation, ensuring resident safety and wellbeing.
- Interdisciplinary Teamwork: Work closely with interdisciplinary teams including physicians, nurses, recreational therapists, and other healthcare professionals to coordinate care plans and maximize outcomes for residents.
- Documentation: Maintain accurate and timely documentation of progress notes and treatment plans in accordance with regulatory standards and organizational policies
