Job Description Summary
The Spiritual Care/Bereavement Coordinator is responsible for the provision of spiritual care services to patients and families/caregivers of Gateway Hospice, either directly or through coordination of care with other spiritual counselors.
Some Duties Include:
1. Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered according to regulatory requirements.
2. Provides direct spiritual care to patients and families/caregivers.
3. Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
4. Connects, re-connects patients with their own clergy when given permission. Documents direct services and ongoing communication with community chaplains and spiritual counselors.
5. Attends and participates in patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group.
6. Participates in on-call chaplain services as needed and requested.
Some Requirements include:
1. Must be an Ordained Minister or a Graduate of an accredited School of Theology.
2. Experience working with death and dying, individuals/family/caregiver preferred.
3. Ability to work as member of interdisciplinary team/group.
4. Comfort in an interfaith setting.
5. Ability to accept different lifestyles, cultures, beliefs, and values.
6. Ability to network with community clergy and congregations.
7. Knowledge of and commitment to hospice philosophy of care.
8. Must be a licensed driver with no restrictions, and have access to an automobile that is insured in accordance with state or organization requirements and is in good working order.
Job Types: Full-time, Part-time
Pay: $45,000.00 - $55,000.00 per year
