```Job Summary```
We are seeking a dedicated and compassionate Service Coordinator to join our team. In this role, you will be responsible for coordinating and managing services for elderly/disabled. The Service Coordinator will work closely with clients, their families, and service providers to ensure the delivery of high-quality and support.
```Responsibilities```
- Develop and implement individualized service plans for clients
- Coordinate and monitor services provided by various agencies and providers
- Conduct assessments to determine client needs and goals
- Collaborate with clients, families, and service providers to develop treatment plans
- Monitor client progress and make adjustments to services as needed
- Provide crisis intervention and support during challenging situations
- Maintain accurate and up-to-date client records
- Ensure compliance with all relevant regulations and guidelines
```Experience```
- Bachelor's degree in social work or related field preferred
- Proven experience in program management or a similar role
- Knowledge of behavioral health principles and practices
- Ability to effectively manage budgets and resources
- Experience working with elderly and disabled.
- Strong supervisory skills with the ability to lead a team effectively
- Excellent relationship management skills to build rapport with clients, families, and service providers
- Demonstrated ability in program development and implementation
- Strong administrative skills with attention to detail
