Pay Rate: $28-$34 per hour
Job Description: Candidate reports to the Community Programs Manager, has knowledge of case management: candidate is well versed in community services available in Delaware as well as various entitlement requirements and application processed. Candidate has experience in training and supervising others in case management work. Candidate will instruct and provide guidance to staff under them to promote efficient services. Candidate will work with senior management in developing case management program, taking the lead in implementing initiatives. Works under the supervision of an administrative supervisor. Oversees and participates in monitoring services provided by team. Interview’s patients, clients, family members and/or persons in a field-based environment deemed necessary to obtain the most complete case history. Assists in identifying, documenting, and advocating for the needs, goals, and problematic barriers that clients are facing. Prepares narrative report writing to document contacts. Will enforce service expectations of assigned staff through employee monitoring, appraisals, and coaching as needed. Partners with community providers as designated by supervisor. Partners with clients to ensure collaboration with current supporters identified i.e. family, friends, clinical home, current treatment services provider, housing supports, etc. to obtain collateral information and ongoing participation in identified recovery journey. Prepares application forms for benefits or services and trains staff in preparing application forms for benefits or services. Assists clients, trains staff in obtaining ID and other needed documentation.
Qualifications:
- 18 years of age or older.
- Have valid driver's license.
- Have basic computer competence in Microsoft Office products; experience working with EMH/EHR preferred.
- Have High School Diploma or GED; college degree preferred.
- Certified Peer Supervisor with experience in Mental Health and Substance Use Disorders strongly desirable.
- Has one or more years supervisory experience: knowledgeable of staff performance evaluation techniques and progressive discipline.
- Willing to work in a field setting and be open to traveling statewide as needed.
- Thorough understanding of case management policies and procedures and HIPAA requirements. and HIPAA laws.
