Service Coordinator Job Description
IMMEDIATE AND MULTIPLE HIRES FOR FULL TIME for the following locations :
Ulster/Dutchess County
*You must live near or around the Tri-state area. This position requires you to conduct field work in NY state.
*This is a full-time hybrid position which requires you to be available Monday thru Friday from 9am to 5pm. Candidates with employment that may interfere with these work days/hours need not apply.
Job Summary
The Service Coordinator/Case Manager assists & maintains the participant maintain waiver enrollment and participation on the TBI or NHTD Waiver Program. The Service Coordinator coordinates all services in their Service Plan. This candidate should be familiar with person centered planning/annual service plans development as well as familiarity with NYS, Social Services , Entitlements & Benefits such as Medicaid, SNAP, SSI/D. The goal is to increase the participant's independence, productivity and integration into the community while maintaining the health and welfare of the individual by coordinating available services.
Service Coordinator Responsibilities
- Act as an advocate for the participant while ensuring their right to choice as outline in their Waiver Participant Rights and Responsibility agreement, while respecting their dignity to risk and the right to fail.
- Assists the participant in the development, implementation of the participants service plan annually.
- Utilize a Person-Centered approach to develop and submit service plans to their Supervisor annually and within the appropriate time frames.
- Provide intensive case management services for up to 24 consumers including but not limited to: crisis intervention, risk reduction and emergency referrals to ensure the participant's health and welfare (*_25th client bonus incentive available_*).
- Service Coordinators contact with the participant will be required at least monthly, with a quarterly in-home face-to-face visit, or more frequently, as needed, and at the discretion of Optimum Choice Services, the participant's choice and/or NYS DOH.
- Schedule and facilitate team meetings bi-annually or as needed for participant's.
- Accompany the participant to appointments as necessary.
- Link the participants with an appropriate service provider.
- Assist participants in the acquisition and maintenance of public benefits e.g., Medicaid, SNAP, Social Security, etc.
- Provide linkage/referrals and coordination of resources/support for medical care, mental health, social, recreational and housing/basic needs, etc.
- Demonstrate flexibility to be available to clients in their homes/communities including working outside of normal business hours as needed.
- Meet all documentation timelines while maintaining quality of services including but not limited to: Complete Service plan packages; Serious Reportable Incidents; Monthly face-to-face documentation and progress notes; Housing requests; Transportation records; and Team Meeting notes.
- Participate in monthly staff meetings and trainings.
- Ensure compliance with all applicable local, State and Federal regulations and agency policies.
- Participate in all mandatory ongoing training and professional development offered.
- Must have a general knowledge and understanding of developmental, physical and cognitive disabilities, traumatic brain injury, chronic medical conditions and/or mental illness.
- Must have valid Driver's or Non-Drivers license is required.
- Candidate must be comfortable with in-home, in-person visits to consumer homes.
- Must have basic computer skills with a home office set-up established with accessibility to a computer (PC preferred). Should have basic knowledge and ability to scan, print, and utilize adobe reader as well as merge PDF files for monthly note submission. Must have internet service with the ability to email. Otherwise remote work option would not be offered and candidate would need to work in office.
- Must effectively communicate both verbally and in writing.
- Must possess excellent time management skills as well as the ability to work remotely and productively.
Position Specifics
- Candidate may have participants across counties. While Optimum Choice Services, Inc. strives to ensure participant's choice, we also recognize the importance of provider accessibility to their client's in emergency situations and will make every effort to provide a caseload with a geographical area in proximity to the Service Coordinator's community.
- Hybrid Position, remote work at home with on-the-road field work required.
Education and Experience
Qualifications
- Masters Degree and one (1) year of experience providing case management/service coordination, information and referral services and service brokering of community based services for individuals with disabilities and/or Traumatic Brain Injury/ or seniors who are medically fragile, OR;
- Bachelor’s degree and three (3) years of experience providing case management/service coordination, information and referral services and service brokering of community based services for individuals with disabilities and/or Traumatic Brain Injury/ or seniors who are medically fragile, OR;
- Associates Degree and five (5) years experience providing case management/service coordination, information and referral services and service brokering of community based services for individuals with disabilities and/or Traumatic Brain Injury/ or seniors who are medically fragile
Any provider currently providing Service Coordination approved pursuant to the qualifications in the waiver prior to the most recent approval is approved to provide services under the new application. New employees/providers are required to meet the new qualifications. The grandfathering of current providers sunsets when the staff person terminates employment with the current employer.
