JOB SCOPE:
Provide peer support/advocacy services to support individuals in their recovery processes. The Peer Specialist will engage individuals on a continuum of care to help improve their health and wellness. Will support individuals from the unique perspective of someone who shares similar experiences.
ESSENTIAL FUNCTIONS:

Conduct in person outreach to clients having difficulty maintaining adherence to treatment (Mental Health/Medical/Substance Use Treatments)to facilitate engagement, and provide bridging and transition support services.

Provide Peer counseling focusing on effective coping skills and life skills.

Participate in the development of recovery plans

Conducts home and community visits, and provide community escorts to appointments

Provide education around recovery.

Promote pro-social behaviors.

Facilitate groups (ADL’s, Recovery Peer Support & others as directed)

Provide linkages and referrals to external social and support services

Participate in Crisis Intervention and Crisis Support Services.
- Participate in outreach events to represent the agency.

Report and record all contact and activities with clients.

Adhere to confidential policy and HIPAA protocol as related to client’s information/interactions.

Attend all agency staff meetings.

Maintain good working relationship with all agency staff, client and visitors.

Performs other duties, consistent with the goals and objectives of the program, as directed by Director.
SKILLS AND KNOWLEDGE:
-
Identify as being actively in recovery from a mental health condition.
-
Bilingual, ability to speak Spanish desirable.

Ensure progress notes are completed within 24-48 hours of the delivery of the service.
EDUCATION AND EXPERIENCE:

High School Diploma or GED
-
NYS Certified Peer Specialist (NYCPS)
