Lutheran Hospice is currently recruiting for a Weekend On Call RN.
We are seeking someone who is a compassionate, dedicated, and caring professional to join our team!
Responsibilities:
- Weekend 1st On Call (call period is 5pm Friday through 8:30am Monday)
- Completes an initial assessment of patient and family to determine hospice needs on weekend admissions or level of care change
- Provides professional nursing care by utilizing all elements of nursing process
- Counsels the patient and family/caregiver in meeting nursing and related needs
- Works in cooperation with the family/caregiver and hospice Interdisciplinary Team Members to meet the emotional needs of the patient and family/caregiver
- Initiates the plan of care and makes necessary revisions as patient status and needs change
- Initiates appropriate preventive and rehabilitative nursing procedures
- Administers medications and treatments as prescribed by the physician in the physician’s plan of care
- Assesses and evaluates patient’s status by writing and initiating plan of care. Regularly re-evaluates patient and family/caregiver needs. Revises the plan of care as necessary
- Provides and maintains a safe environment for the patient
Qualifications:
- Graduate of an accredited school of nursing
- Licensed to practice as a RN in the State of South Carolina with a license in good standing
- Maintain a current CPR certification and valid SC driver’s license
- Minimum two years nursing experience required
- One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care preferred
- Bachelor’s degree, with one year of hospice care experience preferred
- Must have reliable transportation for daily travel throughout the service area
- Must work well within an interdisciplinary team environment as well as function independently at the bedside following policies and procedures and good hospice nursing practices
- Management experience not required but supervisory skills needed. Responsible for supervising hospice aides
- Detailed oriented and strong problem solving skills
- Excellent verbal and written communication skills
