Position: Full-Time Clinical Social Worker
Location: Psychiatry and Behavioral Health, Greenwich Hospital - Yale New Haven Health
Greenwich Hospital, part of the prestigious Yale New Haven Health system, is actively seeking a dedicated and compassionate Clinical Social Worker to join our Psychiatry and Behavioral Health team. Key Responsibilities
- Professional Interaction: Engage professionally with patients of all ages and from diverse backgrounds, including different cultural, socioeconomic, educational, racial, and ethnic backgrounds, genders, sexual orientations, lifestyles, and physical abilities.
- Comprehensive Assessments: Conduct thorough biopsychosocial assessments and/or psychiatric evaluations.
- Treatment Planning: Develop and maintain treatment plans that address patient and family concerns and clinical impressions.
- Patient-Centered Care: Use a patient and family-centered approach to create care plans that meet the needs of individuals and families.
- Education and Counseling: Provide education and counseling services to patients and families.
- Psychotherapy: Utilize best practices to provide individual, couples, family, and group psychotherapy, as well as crisis intervention.
- Collaboration: Consult and collaborate with external providers, agencies, and organizations, including protective services, court systems, outpatient therapists, work/education settings, and housing supports.
- Community Referrals: Provide referrals to community-based services as needed.
- Interdisciplinary Support: Offer consultation and professional support to team members and participate in interdisciplinary rounds to promote integrated care.
- Leadership and Development: Demonstrate initiative in developing services and programs to enhance patient and family care and contribute to quality assurance and performance improvement.
- Documentation: Maintain accurate and timely documentation of social work activities in the electronic medical record.
- Mentorship: Mentor and supervise team members and participate in projects and work groups as assigned.
- Advocacy: Navigate systems and advocate for patients and families.
Qualifications
- Education: Master’s Degree from an accredited school of social work.
- Experience: Experience in treating a broad spectrum of psychiatric conditions, working in a multidisciplinary clinical setting, and training in cognitive-behavioral interventions. A commitment to program development and a biopsychosocial model of understanding and treating behavior and mental health is essential.
- Licensure: Licensed Clinical Social Worker (LCSW).
- Special Skills: Empathy, active listening, critical thinking, strong organizational and computer skills, and the ability to effectively communicate with and advocate for patients and families.
