**Overview**
A Case Manager’s primary goal is to ensure that an individual reaches her optimum level of wellness and functionality within his/her environment. Case Manager’s assess the needs of the individual and family and coordinate care services. This can include counseling, nutrition, rehabilitation, transportation and educational services. Case Manager’salso work to ensure that care is provided in the most comprehensive and cost-effective manner for the individual and his insurance providers. Case Manager’s then monitor the continuity of care and serve as an advocate for the individual.
**Duties**
- Monitor client’s progress through treatment
- Begin working with client upon admission and maintain support through discharge
- Provide support for client through primary therapist direction and treatment planning
- Determine any needs the client may be having while in treatment and report any pertinent clinical information to the treatment team.
- Participate in treatment team meetings to best assure continuity of care between clinical staff and the needs of the client.
- Assist clients with any specialized needs they may have while in treatment i.e. paying bills, family coordination, schooling, finding sober living and IOP programs, and any other direction given from primary therapist.
- Participate in weekly clinical team meetings to gain insight and oversight from the clinical director.
- Complete treatment plans, biopsychosocial assessments and pre-certs under the supervision of a licensed counselor.
**Requirements**
- Valid Driver’s License with clean driving record, as determined by VIrtue’s insurance carrier
- Must hold at minimum a high school diploma
- Minimum 2 years of behavioral health and or case management experience required
- Must hold a minimum of 18 hours of behavioral health related education
- Must be 21 years of age
- Knowledge of Windows, basic email, Microsoft Word, timekeeping software, and basic computer navigational skills.
- Ability to write professional correspondence and reports free from grammatical and spelling mistakes.
- Ability to read, analyze, and interpret emails, letters, policies, procedures, etc.

- Professional and/or personal experience with substance abuse treatment highly preferred
- High level of patience
- Active CPR and First Aid certification required

· Stand, walk and sit during assigned shifts hours.
· Have the ability to manually handle patient belongings with gloved hands.
· Be able to manually assess a patient as part of a physical exam.

· Speak and have access to the special senses within normal limits (hear, taste, smell, etc.) as it applies to patient care.
· Must be capable of lifting up to 20 pounds.
Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
