Hours: Full time, Monday through Friday, 7:30am to 3:30pm
Compensation: Non-exempt, hourly at $20.00 per hour

Job Description Summary:
Under the direction of the Director of Program Services, this position is responsible for all client management including planning, coordinating and executing the delivery of meals to clients / customers of Prescott Meals On Wheels to service area that includes Prescott and Prescott Valley. This includes client documentation, referrals as needed and mandatory reporting requirements in keeping with various contracts and funding sources in accordance with established procedures.
Qualifications:
Associates Degree from an accredited college or university * or* an equivalent combination of education and experience.
2 to 3 years of proven customer service experience sufficient enough to perform required tasks and fulfill the job requirements. A pleasant telephone personality is important as much of this work is done over the phone with clients.
Previous nonprofit and/or case management experience is preferred.
Excellent oral and written communication skills
Proficiency in Microsoft Outlook, Word, Excel, and Publisher
Excellent organizational skills and attention to detail
Excellent interpersonal skills and the ability to work well and closely with a large, diverse population
Must possess a valid Arizona Driver’s License with a clean driving record.
Must be able to obtain a Level One Fingerprint clearance within the first 30 days of employment.
Willingness to provide excellent customer service.
Familiarity with streets and neighborhoods in Prescott and Prescott Valley and ability to use online resources to assign efficient route placement.
Ability to work in a fast paced, often deadline driven environment.
Duties and Responsibilities:
Generate and maintain all necessary paperwork and electronic documentation as it relates to clients who are part of the Prescott Meals On Wheels home delivered meal program in accordance with all regulatory policies and procedures. This includes enrollment of new clients and on-going notations on existing clients.
Maintain detailed and accurate client files with an understanding of the differences between AAA-NACOG, Long Term Care/ALTCS, Private Pay,
Provider Pay and Native American enrollment.
Work closely with the Volunteer Coordinator to make sure all daily delivery routes are covered and that volunteers are adequately trained and all necessary credentials are in place.
Provide individualized case management to Prescott Meals On Wheels clients as well as provide referrals and follow up to other agencies to meet client needs.
Work closely with Case Managers of various providers.
Plan, coordinate and document all homebound meal deliveries working within and around the schedules of clients and volunteer drivers with an eye toward maximum efficiency.
Ensure that all related meal delivery paperwork for weekdays and weekends is accurate and complete.
Prepare and provide monthly reports as needed for billing and other reporting requirements.
On occasion, and when necessary, deliver meals.
Adhere to all organizational policies and procedures.
Seek out opportunities as time allows to accompany volunteer drivers on deliveries to ensure they have a clear understanding of the policies and procedures that govern home delivered meal.
Perform other duties as assigned
