An HCS Direct Care Case Management position has opened with San Antonio’s premier Home and Community Services (HCS) provider and we are seeking a talented new team member for a full time exempt level opportunity.
HCS - CASE MANAGER
We are a 34 year proven Texas based company that provides services to persons with intellectual and physical disabilities in community-based group homes and foster care systems.

Qualifications:
· Bachelor’s degree preferably.
· Solid evidence of 2+ years of progressively advancing team work participation and personal advancement in your most recent past jobs
· Must demonstrate very strong verbal communication skills that we can see will lead to better outcomes for the staff, your coworkers and all persons served. TEAM PLAYER.
· Establish productive work connections with RN's & LVN's to determine the best steps for client's well being.
· Computer skills such as Microsoft Office, operating cellular phones and being able to document client assessments.
· Individual flexibility, commanding communication (oral and written) and interpersonal skills are critical
· Successfully planning and organizing treatment plan development based on client's needs.
· Communicating with direct care staff regarding client wellness and any updates or changes.
· Exemplify outstanding customer service while building relationships with the clients and their guardians.
· Possess an understanding of HHSC rules and standards for the HCS program in State of Texas.
· Possess referral source outreach skills and ability to promote company goodwill with placement coordinators, multiple constituencies and outside survey agency staff including seeking outside natural resource supports for caseload managed
· Willingness to be on-call and respond to after-hours issues when needed
Provide staff training and monitor staff development progress and in-service to promote well-being of persons served on your case load.
Oversee group home settings and coordinate the staffing and with maintenance departments to ensure safe and acceptable living environment
Interface with (verbally, face to face and in writing) State Service Coordinators on the annual development and implementation of Individual Plan of Care and interim updates during the year.
Maintain relationship with and work with a case load of Foster Families

Pay: $40,000.00 - $45,000.00 per year
