Description:
MCHC Health Center is seeking a Remote Triage Nurse to join our team, dedicated to providing compassionate, high-quality care for our communities. As a Remote Triage Nurse, you will deliver telephone triage services within the scope of a California Registered Nurse and organizational nursing protocols. Your role will be pivotal as a clinical liaison, ensuring timely advice, education, and care interventions to address primary care needs effectively.
Primary Duties include:

Supports MCHC’s mission to provide access to excellent healthcare services for the most vulnerable people in our community.

Excellent communication with patients, families, care team, coworkers, and outside agencies. Ability to document information appropriately into patient’s Electronic Health Record.

Determines most appropriate level of care based on assessment and plans appropriate interventions for patients of all age groups. Collaborates with other disciplines when needed.

Successfully completes Electronic Health Record core training and remains proficient within the system. Proficient in essential office technology software to communicate with care team.

Serves as a clinical resource within the Call Center for immediate patient care assistance. Collaborates with Call Center Manager to identify gaps and offer solutions for improvement.

Performs medication refills for patients by utilizing the MCHC LVN/RN Medication Refill protocol. Offers patients appointments and/or communicates with the care provider if patient refill is outside of the protocol.

Monitors patient Emergency Room visits and hospitalizations and schedules necessary follow-up visits as outlined in MCHC’s ER visit/Hospital Discharge policy.

Identifies patient care gaps following MCHC’s Standing Orders protocol, in coordination with MCHC’s Quality Improvement Department. Assists with patient care registries to improve health maintenance activities.

Establishes patient contact to schedule a visit when overdue for a clinic visit, or another type of clinical follow-up as specified by a clinic provider.

Facilitates teamwork within the Patient Centered Health Home team, CC & CI Departments, and exhibits ability to hold team members accountable to achieve overall team excellence.

Communicates with Call Center Supervisor, and Director of Nursing frequently to ensure tasks are accomplished in a timely manner.

Valid State of California Certificate of Registration as a Registered Nurse

Maintain current BLS certificate.

Bilingual preferred

Knowledge of use, administration and effects of Narcotics and Medicines.

Knowledge of Basic Communicable Disease.

Knowledge of Infection Control Practices and Procedures

Knowledge of California Child, Adolescent, and Adult Immunization Schedule

Ability to proficiently use Electronic Health Record
