Position Summary
To achieve a home-like community integrated living arrangement (CILA) based on values of individual self-worth, dignity, equality, and respect for adults with developmental disabilities.
To assist individuals in living their best lives and maintaining maximum independence.
To provide consistent coordination of supports for assigned CILAs.
Education
Must have verifiable documentation of education. Bachelor's degree in human service field or related field that meets requirements for QIDP as defined by the Illinois Department of Human Services with one or more year's experience working with individuals who have developmental disabilities.
Duties
- Writes Implementation Strategies, monthly Q-notes, and other required reports meeting assigned timeframes.
- Completes annual assessments for individuals on assigned caseload.
- Obtains intake and annual consents from individuals and/or guardians.
- Formulates short term objectives and evaluates data to see if individuals are making progress on objectives.
- Ensures desired outcomes identified in individuals’ Personal Plans and Implementation Strategies are being met.
- Maintains charts of individuals on assigned caseload both at the office and in the CILA home.
- Monitors performance of DSPs and discusses opportunities for improvement when needed.
- Monitors documentation for completeness and accuracy and provides retraining as needed.
- Assists in training new Direct Support Professionals.
- Completes QIDP audit twice monthly at assigned houses.
- Responds to crises as required or assigned.
- Leads in such a way as to maximum the independence of individuals and to maintain their rights according to IAG policy and the Department of Human Services regulations.
- Communicates with guardians, Independent Service Coordinators (ISCs), behavior analysts, nurses, and supervisory personnel.
- Completes required documentation accurately and on time.
- Obtains annual CEs as required by DHS.
- Attends appointments or accompanies people to the emergency room or urgent care as assigned.
- Achieves competency or certification in first aid, CPR, Medication Administration and other required skills within prescribed timeframes.
- In conjunction with Benefits Coordinator, assures people obtain and maintain benefits.
- Substitutes for absent co-workers in performance of assigned duties with direction from supervisor.
- Performs miscellaneous duties as assigned by supervisor in order to ensure day-to-day functioning of the home.
- Submits required employee records on time.
- Attends all required trainings and meetings.
- Completes other duties as assigned.
Job Type: Full-time
Pay: $45,000.00 - $50,000.00 per year
