Case Manager (LCM)/ Social Worker (LSW)

As a Case Manager (LCM)/ Social Worker (LSW)
- You will provide comprehensive case management services to residents, ensuring their psychosocial needs are met.
- Conduct resident assessments to develop individualized care plans.
- Collaborate with the Executive Director and care team to coordinate resident care.
- Assist with the coordination of community resources and referrals to enhance resident support.
- Provide support and counseling to residents and their families.
- Ensure compliance with all local, state, and federal guidelines.
- Conduct resident admissions and ongoing assessments.
- Act as a liaison between residents, families, and healthcare providers to ensure a holistic approach to care.

Qualifications:
- You have a minimum of 1 year of experience in social work, preferably in a healthcare or senior living setting.
- Bachelor's degree in Social Work or a related field from an accredited college.
- Experience in long-term care (LTC), assisted living (AL), or skilled nursing facility (SNF) preferred.
- You care about serving seniors.
- You have strong relationship-building skills.
