What you'll be doing:
As a member of our Chronic Care Management (CCM) team, you'll be responsible for using our EMR platform to perform CCM for chronically ill patients. You'll use your health care experience, talents, and problem-solving skills to assess and perform CCM services for patients.
Candidate must be able to pass a drug screen, and background check, have a positive attitude, be a team player, and be willing to learn new skill sets.
Responsibilities include:
- Perform CCM via the CCM software and provide feedback to the Clinical Staff Manager on an ongoing basis.
- Frequent contact with patients to support/manage the Chronic Care Management program to increase positive outcomes.
- Constant communication to and from primary clinician or designee in regards to patient emergent needs and/or life threatening episodes.
- Document all client communications (verbal or written) accurately.
- Demonstrate ability to work with various cross-organizational areas to meet the needs of CCM's clients.
- Expectation that you will become skilled at using technology including the CCM platform, secure email, etc.
- Excellent customer service skills demonstrated by positive feedback from customers and patients.
- Keep your office manager informed of all issues pertinent to the care plan process and any known or perceived issues.
- Contribute as a positive member of the team by supporting all members of the team in a productive and constructive manner.
- Adherence to documentation protocols and best practices for daily work logs, escalation of client issues and internal communications.
Skills and Requirements
- Certified Medical Assistant
- Experience working with Electronic Medical Records (EMR) software as a user is preferred
- Great communication skills
- Ability to build relationships and quickly develop trust with patients and client medical practices
- The ability to thrive in a fast-paced environment
- Highly motivated, results-oriented with strong skills in presenting, communicating, organizing, multi-tasking and project development
- Solid problem-solving and consultative skills
- Excellent computer skills
- Proficient with Outlook
Job Type: Full-time
Pay: $52,000.00 - $58,000.00 per year
