Salary Range: $55.2400 - $84.9400 /hour. Actual compensation may vary based on geographic location, work experience, skill level, and education.
The Clinical Nurse Navigator I is a registered nurse responsible for the seamless coordination of care across the continuum for patients with complex, integration sensitive diagnoses. The Navigator is a patient advocate who coordinates care in conjunction with the physicians from time of diagnosis or admission through discharge or up to end of life. Educates patient and family/significant others on the health care experience; implements, coordinates, monitors and evaluates care options, services and resources. Links family/patient to appropriate resources as needed. Proactive in finding ways to enhance the patient experience clinically and in service. Provides an exceptional patient experience. The Nurse Navigator assists in quality improvement initiatives by gathering, tracking, and researching quality metrics. Monitors outcomes, e.g. patient satisfaction, compliance with treatment regimen and loyalty. Interacts with department Nurses, may orient and train incoming Clinical Nurses and Patient Care Assistants. Performs other duties as assigned.
Education and Experience
Minimum of 5 years’ experience in nursing and 3 years’ of experience in area of specialty required.
License Required
Registered Nurse in the State of California - Current and in good standing.
Certifications Preferred
Current BLS certification.
