The WV Bureau for Public Health, Office of Epidemiology is currently seeking four Regional Linkage to Care Specialists in the northern, eastern and southern area of West Virginia. This a temporary position with the possibility to obtain permanent, full-time employment.
4 positions open in Berkeley, Kanawha, Cabell and Harrison counties.
Position Summary
The Regional Linkage to Care Specialist will work with the Ryan White Medical Case Managers, Disease Intervention Specialists and other community partners and stakeholders to facilitate linkage to care activities and to strengthen collaboration among key linkage staff at health departments, local HIV care providers, medication assisted treatment (MAT) sites, substance abuse treatment centers, emergency departments, hospitals, and jails to ensure a comprehensive referral network for expedient linkage to care and rapid initiation of anti retro-viral therapy (ART). Will assist newly diagnosed HIV positive individuals, or those that have fallen out of care with getting to viral suppression of HIV infections. Will assist with cluster and outbreaks linking STI and HIV positives to care.
Minimum Qualifications
Training: Bachelor's degree from a regionally accredited college or university.
Substitution: Additional experience as described below may be substituted for the required training on a year-for-year basis.
Experience: Two (2) years full-time, equivalent part-time paid or volunteer experience in a technical or program area that is related to the area of employment.
Substitution: Post-graduate education in a field related to the area of employment may be substituted for the required experience on the basis of fifteen semester hours for one (1) year of experience.
OR
Master's degree in social work from a regionally accredited social work program from a regionally accredited college or university.
