Case Manager for Coordinated Intake and Outreach Youth and Adult (CIO)

Purpose: To implement and evaluate components of the Coalition’s programs as they relate to the delivery of direct services under each program’s specified criteria. It serves to engage people experiencing homelessness in agency services by implementing all outreach services developed by the organization. Interfaces with all new applicants to determine eligibility under multiple programs within the organization; once eligibility is established, follow with a direct placement into a case manager’s schedule in a manner that adheres to all protocol, organizational goals, program requirements, and monitoring processes. With the Veteran population, this position will assist with health care navigation.
To facilitate the successful progression of programs focused on homeless and at-risk youth and young adults aged 18-24, including unaccompanied youth and pregnant/parenting youth as they pertain to the YHDP programs under the direction of the Coalition. This position encompasses areas of focus pertaining to direct service delivery and programming support. All areas support the four core outcomes of the Coordinated Community Plan (CCP) for the target population-stable housing, permanent connections, education/employment, and social-emotional well-being. The main objective is to implement and assist in evaluating components of the Coalition’s programs related to the delivery of direct services under each program’s specified criteria. Serves to engage the homeless in agency services by implementing all outreach services developed by the organization.
Interfaces with all new applicants to determine eligibility under multiple programs within the Housing First and Coordinated Entry process, followed by direct placement into a case manager’s schedule in a manner that adheres to all protocol, organizational goals, program requirements, and monitoring processes. With the youth-targeted population, this position will work in tandem with the Youth Coordinator and Housing Case Manager to implement client-centered assessment and navigation, helping youth and young adults to determine their individual pathway from at risk of or literal homelessness into self-sufficiency and stability.
I. ESSENTIAL TASKS OF THE POSITION (The following is intended to be illustrative, not all-inclusive):
1. Perform individual and family assessments (Intake) to begin the transition from homeless to independence by identifying specific needs and conditions of both the individual and the household for those sheltered and unsheltered.
Identify and offer supportive services specifically for youth, including special populations, which incorporate harm reduction, trauma-informed care, and positive youth development. Create a system to connect youth to the available resources (juvenile justice-involved youth, runaway youth, minors, victims of sex trafficking and exploitation, LGBTQ+, and gender non-confirming youth) by designating members of the community as a contact person/ outreach representative–someone to guide youth to the process (Guidance counselors, social workers, therapists, CPS workers, Court advocates, POs) covering any agency where the youth present themselves
2. Provide expedient follow up on pending application verifications that can present a barrier to quickly resolving homelessness
3. Internally refer clients to the correct programs based on the outcomes of outreach and centralized intake assessments
4. Monitoring clients in a comprehensive outreach effort to ensure the organization is identifying and successfully engaging the unsheltered homeless and the at-risk homeless; completing intake on all sheltered homeless to reduce side door admissions, receiving quick and successful referrals from community partners
5. Work with Veterans to provide all require health care navigation services via the SSVF program to link to client to VA and non-VA based health care
6. Assist the client by developing, implementing, and regularly revising program specific elements including but not limited to:
Document resources utilized and needed for each applicant and furnishing linkage and referral to all external and internal services and support identified through the screening process
Providing advocacy to ensure continuity and integration of services
Monitoring and documenting progress for each case
Provide crisis support services with direct crisis intervention, as needed, including but not limited to life-saving measures
Assisting with all aspects of ending and preventing homelessness within the confines of our organization’s mission and abilities
Issuing correspondences regarding discharge planning protocols
Maintaining and safeguarding the organization’s Non-Admission list with the Blood Borne Pathogen documentation and notifications
7. Review, maintain and implement all appropriate program service policies and procedures. Keep informed of applicable regulations and guidelines for the programs

8. Assertively promote Coalition programs by developing positive alliances with staff, landlords, community partners and financial stakeholders that result in the improvement of housing and employment opportunities for the clients
9. Assist in the coordination of work activities of all programs within the Coalition or outside agencies to prevent delays in actions required or to improve services to clients or agency operations
10. Maintain a currently revised community resource guide and the professional contacts of the organization
11. Work efficiently with other personnel to assist clients in the removal of barriers to ending homelessness, such as applying for and transportation to access vital records, retrieving necessary supplies for housing and making appropriate follow up appointments for housing or health care related matters
12. Learn the language of resolving homelessness and implement adopted practices, such a diversion, discharge planning adherence and verification of homelessness when necessary enroll the client in services
13. Provide information, referral, and follow-up to local service providers to ensure that program participants are linked to appropriate supportive services to achieve self-sufficiency
14. Develop linkage with outside agencies to schedule monthly training workshops for clients in various disciplines such as: homemaking skills, credit counseling and budgeting, educational and job opportunities, nutrition, parenting, health care, childcare, support services, homebuyer education, and entrepreneurial business development
15. Advise potential clients and participants on the mission and goal of the organization and how they pertain to the person’s self-improvement plans
16. Prepare all reports on program status, track aggregate data on program achievements, and prepare annual reports for submission to funding sources and supervisor, as requested. Communicate to the Executive Director on all data generated from reporting methods in both written, narrative form and verbally
17. Assist in the establishment of program goals and objectives for review and approval
18. Coordinate with all entities as necessary to carry out the program goals while understanding that the organization is the client you represent
19. Work with a Case Management team to ideally select house partners for the Permanent Supportive Housing Program that will promote healthy living practices and reduce instances of crisis and conflict
20. Ensure that that Coalition utilizes the HMIS system by adding clients to the database in a point in time fashion, assuring that reports are being printed with accurate data, that fields of data capture the necessary demographic for reporting, that all issues are immediately communicated to the HMIS Specialist and overall that the system’s capacity for usefulness is fully maximized by staff in the CIO position

III. POSITION REQUIREMENTS AND QUALIFICATIONS: A. Education Level:
Bachelor’s degree OR Associate’s Degree with direct experience with the population OR a High School Diploma with extensive experience coupled with a proven level of competency in providing the necessary duties
B. Experience in Related Field
1. Experience in addressing the needs of homeless families or individuals, or other subpopulations of those who are marginalized and are presented with barriers
2. Experience in working in the non-profit, community health or social services sector
3. Possesses and presents to the Executive Director an extensive knowledge and an in-depth understanding of the organization and its practices

12. Maintain confidentiality of client information, agency processes, programs, and data, which would be damaging if not properly safeguarded.
