CalAim Case Manager: We seek a Case Manager Housing Specialist and Crisis Intervention Housing Specialist for our Cal AIM program.
Shifts Available: Full-Time, Traditional Work Hours, Non-Exempt
SUMMARY
The Case Manager will travel to multiple locations, primarily in the Orange County area, to manage cases. This includes working with clients to provide quality service, assist with attaining housing recovery support, and ensure clients get the needed services.
The Case Manager’s primary responsibility is to reach and engage new clients, including coordinating housing client support with mental and primary health recovery services and ensuring community integration.
The Case Manager's responsibilities will include heavy customer service, good people skills, and the expectation to use the harm reduction model to maintain high-quality services while fostering collaboration between clients and programs. This person must build strong relationships with our partners and service providers and ensure clients receive the best care possible.
ESSENTIAL DUTIES & RESPONSIBILITIES
Conducting intake and needs assessments
Initiate contact-building caseloads-identify program qualification
Collaborating with clients and other Cal-AIM staff to develop Client Treatment Plans (CTPs)
Providing ongoing case management services to support CTP goal completion, including, but not limited to:
Assistance with benefits establishment
Assistance with obtaining identification documents
Employment linkage and assistance
Assistance with housing navigation and application
Providing referrals and linkages to substance abuse, primary, and mental health care services
Providing a high-quality customer service environment for all clients and guests
Maintaining documentation standards as set forth by the program contract and Homeless Intervention Service of OC
Coordinate services with partner agencies as needed and provide assistance with the completion of forms, including Disability and Medi-Cal eligibility, when applicable.

Conduct in-home client visits and model a positive attitude.
Assists clients in improving home management skills, family coping parenting skills, self-esteem, anger management, and stress management.
Travel with the tenant to appointments when necessary.
Participate in weekly team meetings as requested.
Attend all required training to ensure individual and professional growth. • Answer telephones and respond to email inquiries in the office.
Assist with furniture purchases, linkage to other supportive programs.
Provide a Full wrap around approach to empower client to be housed.
QUALIFICATIONS
- Required: A bachelor’s degree from an accredited college or university in Psychology, Social Work, Human Services, or a related field OR two years of experience providing services to Homeless populations.
- Previous administrative experience preferred.
- 1-2 years of experience working with homeless and/or low and mixed-income populations, in addition to experience in child welfare, family work, and substance abuse
- Have an understanding of low-income and homeless individuals and their specific needs
- Knowledge of crisis prevention, intervention, goal setting, and resolution techniques; should be able to match such techniques to particular circumstances and individuals
- Previous administrative experience preferred
- Proficient skills in Microsoft Office, including Microsoft Word and Excel
- Excellent customer service, communication and problem-solving skills

CA driver’s license or equivalent.
Special Requirements
This position requires the incumbent to work sporadically on evenings or weekends for events as needed. The incumbent must also successfully pass a background check.
This position participates in weekly meetings with Management and one-on-one sessions with the Program Manager/Director.
Working Conditions
This position requires walking and climbing up and down stairs. The candidate must also be able to lift 20 pounds frequently. The candidate must be able to work both indoors and outdoors in a variety of weather conditions. The candidate must also be able to work in an office building setting with a moderate noise level.

Pay: $21.00 - $25.50 per hour
