The City of Alameda Housing and Human Services Department (HHS) is seeking a case manager professional with experience serving populations struggling with mental health, addiction, housing insecurity and homelessness. This person will work alongside HHS staff and social service providers to deliver direct support to constituents experiencing or at risk of experiencing homelessness. The position's roles include: direct constituent interaction in the office, street and at constituents’ homes and places where they receive services; accompanying individuals to connect with services when and if necessary; interfacing effectively with social service providers to triage cases and address unmet needs; system navigation; client advocacy and record keeping and administrative functions associated with securing social services. We’re looking for a person with a compassionate, customer service approach who can meet people where they are at.
The ideal candidate will embrace HHS' emphasis on person-centered driven problem solving, have a proven record of taking initiative, and be passionate about helping marginalized populations in effective ways, with an emphasis on clear communication and consistent follow up to achieve client-driven solutions.
