Job Overview:
We are seeking a dedicated and experienced Clinical Pharmacist to join our team in providing Medication Therapy Management (MTM) services. This is a full-time, remote contract position that allows you to work from the comfort of your home while making a significant impact on patient care. The ideal candidate must be licensed in the state of Florida and fluent in Creole to effectively communicate with our diverse patient population.
Key Responsibilities:
- Medication Therapy Management: Conduct comprehensive medication reviews, identify potential medication-related problems, and develop individualized care plans.
- Patient Education: Provide education and counseling to patients in Creole to ensure they understand their medication regimens, potential side effects, and the importance of adherence.
- Collaboration: Work closely with healthcare providers to coordinate care, optimize medication therapy, and improve patient outcomes.
- Documentation: Maintain accurate and thorough documentation of all patient interactions, assessments, and interventions in accordance with regulatory and company standards.
- Quality Improvement: Participate in quality improvement initiatives to enhance the MTM program and overall patient care.
- Compliance: Ensure all activities comply with state and federal regulations, as well as company policies and procedures.
Qualifications:
- License: Active and unrestricted pharmacist license in the state of Florida.
- Language Skills: Fluency in Creole is required.
- Education: Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
- Experience: Minimum of 2 years of clinical pharmacy experience, preferably in a MTM or ambulatory care setting.
- Communication: Excellent verbal and written communication skills in both English and Creole.
- Technical Skills: Proficient in using electronic health records (EHR) and other healthcare-related software.
- Interpersonal Skills: Strong ability to build relationships and work collaboratively with patients, caregivers, and healthcare providers.
