Under the general direction of the Clinical Director, this position will be responsible for maintaining the orderly delivery of treatment for YAS clients.
Case Managers are directly responsible for a caseload, with clients ranging in age from 5 to 21. The Case Manager must have the capability to deal with frustration and conflict. Case Managers must exercise sound judgment and conduct their professional life as a socially appropriate model for YAS clients. Case Managers must demonstrate proficiency in the accurate and timely completion of treatment documents and must perform these duties with limited supervision.
The Intake Coordinator is responsible for retrieving all referrals that come into the agency for all programs. The Intake Coordinator will need to be able to communicate effectively with a variety of referral sources around the state of Alaska. The Intake Coordinator will manage the orderly flow of initial treatment documentation for new clients, as well as tracking paperwork and communication regarding all referrals.
Essential Job Functions
Case Manager
· Understands and effectively engages the clients with the treatment program.
· Communicates effectively with a wide range of children and families.

· Schedules daily activities for assigned caseload and prompts clinician to maintain timelines on clinical assessments and reviews, psychiatric assessment information integration, and crisis intervention documentation.
· Coordinates with other service providers when necessary to facilitate the individualized treatment planning process.
· Documentation is in line with agency policies and state and federal regulations.
· Ensures that all necessary appointments related to treatment are scheduled in a timely manner.
· Understands a coordinated, multidisciplinary team approach to engaging clients and families.
· Implements and monitors clients’ treatment plans.
· Develops personalized treatment plans for each client to address their individual needs.
· Completes and maintains treatment documentation in a timely manner including note templates, progress notes, functional assessments, treatment plans and treatment reviews.
· Schedules, documents and attends all treatment team meetings for assigned caseload.
· Understands and maintains client confidentiality and treatment team collaboration.
· Attends weekly staff meetings.
· Perform other related work as assigned.
Intake Coordinator
· Receive referrals for client services from various sources, including faxes, emails, and phone calls.
· Review referrals for relevant information, and screen applicants on their eligibility for each program company wide.
· Communicate effectively with the community, as well as state-wide partners to develop referral relationships.
· Schedules interviews with potential clients and guardians and assists in the completion of admission paperwork.
· Schedules travel through Medicaid for admitted clients.
· Tracks paperwork and communication regarding all referrals and keeps information on file for previous referral information requests.
Knowledge, Skills, and Abilities
· Knowledge of the Alaskan Native culture and population
· Knowledge of the AKAIMS database, or the ability to learn about it
· Must have excellent organization and communication skills
· Must have excellent customer service skills, and the ability to calmly discuss sensitive situations with potential clients and their families
· High attention to detail
· Sufficient awareness of and appropriate application of developmental theory
· Utilize MS Word and Excel to automate administrative tracking and data collection
· Ability to complete other duties as assigned
Licenses and Certifications Needed
· Current driver’s license
· CPR/First Aid
Minimum Qualifications
· High school diploma or Bachelor’s degree (preferred) with an emphasis in human services.

Pay: $21.00 - $24.00 per hour
