The Beneficiary Alignment Specialist plays a vital role in supporting the ACO Reach program's objectives of improving patient outcomes and promoting healthcare quality and efficiency. By fostering collaboration between patients, providers, and healthcare facilities, the specialist contributes to a patient-centered approach to care delivery. This position directly reports to the Director of Nursing.
Collaborate with ACO Participant Providers, the Intrinsia team, and healthcare facility
staff to identify patients eligible for alignment within the ACO Reach program.
Explain benefits and goals of alignment to patients and families and obtain their consent
for participation.
Liaise with healthcare facilities to ensure seamless coordination of care.
Communicate patient's needs and preferences to healthcare facility staff to optimize the patient experience.
Participate in ongoing training and development activities to enhance knowledge of ACO Reach alignment strategies and best practices.
Attends routine ACO meetings and monthly all staffs.
Provide feedback and suggestions for program improvement based on observations and experiences in the field.
Align patients via hardcopy of voluntary alignment form and via QR code to appropriate Participant Providers organization.
Educate on Benefit Enhancements of the ACO Reach Program and have knowledge of the contracted Preferred Providers within the program.
Follow up and keep patients and families informed on patients status of alignment.
Observe the ACO Reach compliance guidelines when aligning patients to the ACO reach program.
Follow up with patients and families after a Participant Providers visit with patient.
Collaborate and work as a team with the ACO Reach Preferred and Participant Providers.

1. Possesses an ability to maintain good interpersonal relationships with patients, families, coworkers, and other health team members.
2. Expert customer service ability and a strong desire to perform as an integral part of the care team.
3. Ability to work independently and collaboratively in a fast-paced, dynamic environment.
4. Entry level sales and marketing, 0-2 years of experience.
5. Knowledge of ACO healthcare regulations and compliance requirements related to patient privacy and consent, preferred.
