Avatar Healthcare Inc. is seeking immediate hire of PT/FT Home Health RNs. The primary functions of the Home Health RN are to coordinate patient care with the interdisciplinary team including physicians, family, and referring agency: administer skilled nursing care for clients in their place of residence, and assume the responsibility for coordination of care.
-Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patient's physical, functional, psychosocial, and environmental needs and ADLs, i.e., the risk for grief, cultural and spiritual, verbal and non-verbal.
-Implement the individualized POC and recommend revisions to the plan as necessary.
-Consulting with and educating the patient/family regarding disease process, self-care techniques, ---end-of-life care, nutrition, and dietary needs. Providing training to other staff as needed.
-Initiating appropriate preventive and rehabilitative nursing procedures.
-Preparing clinical and progress notes that demonstrate progress toward established goals.
-Coordinating all patient/family services and prioritization of needs with the members of the IDT.
-Use of case management approach and refer to other services as needed.
-Informing physician and other personnel of changes in the patient’s needs and outcomes of the intervention, while evaluating patient and family response to care.
-Determining the scope and frequency of services needed based on acuity and patient/family needs.
-Assessing the ability of the caregiver to meet the patient’s immediate needs upon admission and throughout care.
-Evaluating own needs for support and using identified systems to meet the need.
-Communicating information using the current process and technology available to the organization.
-Applying specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient’s eligibility.
-Perform on-call responsibilities and provide on-call service to patients/families as assigned.
Job Qualifications:
Education:
Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
Licensure:
Current license as a registered nurse (RN) in Texas or in accordance with the Board of Nurse Examiners rules for Nurse
Licensure Compact (NCL), current Texas Drivers License.
Experience:
Minimum one year of experience as a registered nurse obtained within the last 36 months. Minimum of two years of hospice experience preferred. Management experience preferred.
Skills:
Nursing skills as defined as generally accepted standards of practice. Excellent interpersonal skills and ability to communicate effectively. Demonstrates proven decision-making skills. If required to make visits, needs proof of current CPR.
Must read, write and comprehend English.
Transportation:
Reliable transportation. Valid and current auto liability insurance.
Environmental and Working Conditions:
Performs duties in an office environment during agency operating hours. If required to make patient visits, may work in patient’s home in various conditions; possible exposure to blood and bodily fluids and infectious diseases; ability to work a flexible schedule; ability to travel
locally; some exposure to unpleasant weather; PRN emergency call, including death pronouncement. Must be available at all times either in
the person or via telecommunications.
Essential Functions:
-Directs and coordinates clinical departments and sites; assumes responsibility for continuity, quality, and safety of services delivered in compliance with state and federal regulations. (Conditions of Participation).
-Promotes the philosophy and administrative policies of the Agency, in an effort to ensure the quality of patient care.
-Supervises and provides direction to subordinates, in an effort to ensure quality, compliance with the -- Plan of Care, assessment, and reassessment of patients’ needs and continuity of services by appropriate health care personnel.
-Monitors the QAPI Program and assures appropriate corrective measures are performed.
-Promotes compliance with all fiscal intermediary and/or other third-party payors, through education, coaching, and other assistance as necessary.
-Participates as a member of the Interdisciplinary Team (IDT),
-assists in the development of the IDT plan, and participates in QAPI activities.
-Meets mandatory continuing education requirements of the Agency and licensing board.
-Promotes and educates regarding the concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
-Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members.
REQUIRED KNOWLEDGE & SKILLS

- At least one year of Kinnser EMR charting
