Open Door is seeking a Specialist, Youth Treatment & Recovery located out of our Purchase, NY location. The Specialist, Youth Treatment & Recovery will provide ongoing care coordination support to patients enrolled in the Family Tree Program which is an initiative to enhance comprehensive treatment and recovery services for vulnerable adolescents and transitional aged youth with substance use disorders (SUD) and/or co-occurring mental health disorders (COD). Following the patient-centered medical home health care delivery model, the Program Specialist participates in a patient-focused, team-based approach to provide direct and collaborative care coordination. The services provided by the Program Specialist aims to improve patient health outcomes and assist patients in taking control of their health care and becoming independent, and informed consumers of health care, substance use treatment and mental health services. Primary responsibilities include providing support to Care Managers and patients through activities such as data entry/documentation, transportation, and appointment scheduling/coordination, and obtaining records from external service providers.
RESPONSIBILITIES
- Schedules appointments (internal and external)
- Coordinates and schedules transportation
- Obtains records from external service providers
- Identifies patients with gaps in care and/or due for follow up care and assists with scheduling (e.g., cancer screenings)
- Sends correspondence (i.e., letters, text messages) to patients and responds to inquires via the Patient Portal
- Assists with completion of and follow-up on applications
- Enters and collects data for internal and external systems, including patient document scanning and upload
- Monitors and assists with addressing high priority telephone encounters assigned to team members who are out of the office/unavailable
- Provides in-person coverage as needed
- Assesses patients’ needs related to the social determinants of health and makes appropriate and timely referrals to address identified needs
- Provides assistance related to low-cost pharmaceutical drug programs through tasks such as communicating with pharmacies, medical providers, and providing options to patients enrolled in the Family Tree program
- Identifies patients in need of insurance assistance and makes referrals to Insurance Enrollment Department or care coordination staff for assistance.
- Outreaches patients who have had an ED visit and coordinates follow-up care
- Travels as required to community-based meetings, medical/behavioral health provider sites and meetings with Family Tree enrollees
- Represents Open Door at community health fairs and community outreach activities
- Outreaches patients to conduct surveys
- Collaborates with the Family Tree team to ensure program and data reporting requirements are met
- Informs direct supervisor of any problems that may affect quality and standards of patient services
- Fosters good communication between Open Door and patients by returning all patient telephone requests in a timely manner
- Demonstrates ability to function in a culturally sensitive manner within Open Door’s philosophy and mission
- Attends staff and other organizational meetings
- Participates in staff development programs, educational activities, seminars, and training opportunities
- Maintains patient confidentiality at all times per Open Door policies and procedures
- Adheres to National Patient Safety standards as defined by the Joint Commission on Accreditation of Healthcare Organization and the Open Door Family Medical Centers
- Prepares and maintains documentation in compliance with Open Door and Joint Commission standards and applicable city, state, and federal laws
- Meets annual mandatory training requirements
QUALIFICATIONS
- High school diploma or associates degree required; college degree preferred
- Experience in substance use treatment, mental health, healthcare and/or social services settings preferred
- Experience with electronic medical records is preferred
- Knowledge of/experience in care coordination/case management is preferred
- Fluent in English and Spanish
- Strong interpersonal skills including excellent active listening and clear oral communication skills
