Summary
According to social work practice and professional standards, works as a member of the health care team to facilitate continuity of care by assessing psychosocial needs and, planning and coordinating discharge follow-up and care appropriate to the patient’s level of care. Provides supportive counseling and crisis intervention as needed. Provides patients and their families with resources regarding housing and placement, legal issues and referrals.
Minimum Qualifications
- Master’s degree in Social Work (MSW).
- Current licensure in the Commonwealth of Pennsylvania as a Social Worker, or license eligible. Must receive license within six months of hire.
- Strong verbal and written communication skills.
- Ability to work independently and to gather pertinent patient information.
- Fundamental understanding of personal computers and familiarity with the Windows operating system.
- Strong negotiation skills and the ability to address complex issues with patient/family and staff across multiple departments.
Preferred Qualifications
- One year of psychiatric inpatient experience in a social work position.
