Duties and Responsibilities:
PROVIDE NURSING CARE TO RESIDENTS/PATIENTS IN ACCORDANCE WITH THE PLAN OF CARE, NURSING POLICIES AND THE STANDARDS OF NURSING PRACTICE INCLUDING OVERSEEING THE DELIVERY OF CARE BY NURSING ASSISTANTS:
- Receive and give Inter-shift report, both verbally and in writing by completing the 24-hour report
- Assist in assigning duties to nurses aides based on resident, unit needs and staff's abilities
- Make rounds at the beginning and end of the shift on the entire unit to observe residents and identify any immediate needs
- Take and record vital signs (temperature, pulse, respiration and blood pressure); report abnormalities to physician and Nurse manager or designee
- Assist in the development, updating, evaluating and modifying of the comprehensive care plan for the residents on a timely basis according to schedule or change in condition
- Complete medication administration according to medical provider's orders and within the guidelines established by the Pharmacy Committee and the standards of nursing practice
- Perform treatments as ordered by the medical provider within acceptable guidelines and standards of nursing practice and according to the nursing policies and procedures
- Obtain specimens for lab tests as ordered
- Inform nursing personnel of new admissions, their expected time of arrival, room assignment, etc.
- Ensure that rooms are ready for new admissions
- Greet newly admitted residents upon admission, escort them to their rooms as necessary
- Participate in the orientation of new residents/family members to the facility
- Make rounds with medical provider
- Requisition and arrange for diagnostic and therapeutic services, as ordered by the physician, and in accordance with our established procedures
- Inform nursing personnel in verbal report of changes in condition of resident
- Monitor seriously ill residents and ensure prompt communication with the physician and family when a change of condition occurs
- Assist in making skin care rounds weekly and complete the documentation on the flow sheets and CCP
- Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit
- May attend interim and regularly scheduled care plan meetings
- Carry out resident teaching as part of the plan of care
- Begin pre-discharge education program for the resident/family according to discharge plan
- Carry out restorative and rehabilitative nursing program to promote independent functioning of residents
- Makes determination of death as assigned
PERFORM ADMINISTRATIVE DUTIES, CHARTING AND DOCUMENTATION INCLUDING COMPLETING MEDICAL FORMS, REPORTS, RECORDING IN THE MEDICAL RECORD IN A TIMELY MANNER AND IN ACCORDANCE WITH ACCEPTED STANDARDS OF PRACTICE AND FACILITY POLICIES, PROCEDURES AND THIRD PARTY PAYOR REQUIREMENT:
- Ensure that nursing treatments and services such as dressings, catheterizations, tube feedings, suctioning, respiratory care, trach care, vital signs, and ADL activities are carried out in accordance with the resident's plan of care, accepted standards of practice and Citadel Rehabilitation and Nursing Center at Kingsbridge policies and procedures
- Assist in admission, transfer and discharge process of residents
- Chart nurse's notes in an informative and descriptive manner that reflects the care provided to the resident and the response to care
- Ensure that reports are completed in a timely manner that are required by the facility, governmental agencies and other agencies as needed in the interest of resident care
- Attend clinical meetings for the unit as scheduled or requested
- Participate in facility surveys (inspections) made by governmental and other agencies
- Demonstrate an ability to recognize problems and develop solutions
- Contribute to cost management by effective and economical use of staff, supplies and equipment
- Fill out and complete reports of accident/incidents involving residents, staff or visitors
- Perform routine charting duties as required and in accordance with facility policies and procedures
- Sign and date and time all entries made In the medical record
- Count narcotics with a second nurse at the beginning and the end of the shift and sign for the count
- Complete the 24 hour report
- Be responsible for the exchange of narcotic keys at the end of the shift
- Ensure that all visitors to the unit and staff are properly identified
- Report to immediate Supervisor all reports of suspected abuse
- Assist with the completion of scheduled assessments utilizing the MDS 3.0
ENSURE RESIDENTS' SAFETY IN ACCORDANCE WITH RESIDENT SAFETY PROGRAM:
Follow safety features of the plan of care
Keeps residents bedside unit and surroundings clean, safe and noise free as possible
Receives reports and/or makes reports of unsafe equipment and/or conditions to the nurse manager or RNS
Complete emergency equipment/supply checks each shift
Report malfunction of equipment and/or broken furniture immediately to Maintenance Department
Participate in the implementation and maintenance of the Infection Control program for monitoring and/or reporting communicable and/or infectious diseases among resident and personnel
Ensure that all personnel performing tasks that involve potential exposure have attended the appropriate in-service training prior to performing such tasks
Ensure that the assigned nursing personnel follow established infection control procedures when isolation precautions become necessary
Ensure that personnel follow the established procedures for the use and disposal of personal protective equipment
Ensure that all residents are wearing appropriate identification bracelets
Identify the resident by reading the ID band and checking the photograph in the MAR
Ensure medication is given to the right resident by checking all resident have the appropriate identification including wristbands before dispensing notification
When administering medication, remain with resident to ensure that he/she swallows or takes medications as instructed
Follows standard list of abbreviations of medical terminology that are used in providing care
Document in the MAR with initials, in the appropriate time and schedule
Clear medication chart upon completion, replenish supplies, return cart at the nursing station and ensure it's locked and no medication are accessible
Read back medical orders given over the phone including the name and spelling of the drug, dosage, route, frequency and reason
Assist in developing safety standards for the nursing service department as necessary.
Ensure that all visitors to the unit and staff are properly identified
Otherwise adhere to resident safety program while performing day to day responsibilities
Report observations that are inconsistent with resident safety program or violations of the policy to immediate supervisor
PARTICIPATE IN AND ADHERE TO EMPLOYEE HEALTH, SAFETY, SECURITY AND CORPORATE COMPLIANCE PROGRAMS AND ACTIVITIES:
- Receive annual physical including appropriate screening for tuberculosis
- The ability to conduct all activities within the resident abuse prevention manual
- Cooperate with investigations regarding employee health, safety, security, and corporate compliance programs
- Report all accident and incidents to Supervisor immediately upon occurrence
- Correct unsafe or hazardous condition encountered, if unable to correct, report condition to immediate Supervisor upon discovery
- Conduct all activities within established safety, security and infection control procedures and guidelines
- Conducts all activities within established corporate compliance policies and procedures
- Observe and respond to all standing Citadel Rehabilitation and Nursing Center at Kingsbridge security precautions, devices and alerts as they pertain to work and common area
- Respond to fire/disaster drills and emergencies
- Operate equipment and machinery in accordance with manufacturers' directions and Citadel Rehabilitation and Nursing Center at Kingsbridge procedures
PARTICIPATE IN MANDATORY IN-SERVICE AND JOB TRAINING PROGRAMS:
- Attend mandatory in-services annually
- Attend in-services, workshops and seminars to further professional development Participate in orientation of new employees
- Identify learning needs of the nursing staff

JOB COMPETENCY:
- Must be a graduate of a RN nursing program
- Current registration and license to practice as a Registered Nurse in the State of New York
- One year of clinical nursing experience
- Must be CPR/BLS certified
- One year experience in geriatrics preferred
- The ability to read, write, speak and understand the English language, knowledge of other languages helpful Has effective oral and written communication skills
- Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of the position can be fully met
- Must be able to use a computer or use prosthetics or devices that will enable such use
