- Conduct initial assessments of all people who receive health services and reassess at any time that there has been a significant change in the person’s health status, including re-assessment within 24 hours after any significant injury or illness.
- Develop a Nursing Care Plan for each individual on the nurse’s caseload, and update the plan as individual health needs change.
- Perform all services in accordance with the Maryland Nurse Practice Act, Comar 10.22 (Regulations governing services to individuals with disabilities) and Emerge policies and procedures.
- Conduct 45-Day Assessments/Reviews of any individual who is receiving a delegated nursing function.
- Review any paid staff person (at least semi-annually) who is performing delegated functions to ensure that they are performing those functions in accordance with rules, regulations, and in the best interests of the consumer.
- Review Medication Administration Records prior to distribution each month, review of completed Medication Administration Records, and ensure that each medication record is accompanied by a current Physician’s Order Sheet and properly labeled prescription at all times.
7. Act as a liaison with physicians and other health care professionals.
8. Attend Interdisciplinary Team Meetings of all individuals when requested and where participation is essential to the development of overall health services.
9. Provide Quality Assurance that all individuals are receiving health services and take corrective action when necessary.
10. Promote independence in health care and wellness for all individuals.
11. Provide training to all individuals with disabilities served, as appropriate, to increase their independence.
12. Document all interventions as approved by Emerge.
13. Complete and remain current in all training at Emerge including Principles of Community Integration and Inclusion.
14. Complete initial nursing assessments on new individuals within 24 hours.
15. Complete nursing assessments on those individuals discharged from the hospital with 24 hours.
16. Train and educate employees on diets
17. Be on call for health emergencies through cell phone communication.
18. The nurse must posses exceptional organizational skills and be able to work independently.
19. Other duties as assigned.
