Elder Care Management is the leading Aging Life Care provider serving the greater Sacramento region including Sacramento, Placer, El Dorado, and Yolo counties. We are excited to be seeking qualified professionals to add to our amazing team of Care Managers. This individual will work directly with our older adult clients and families to provide guidance and support for our clients’ aging journeys. Duties & Responsibilities
- Conduct regular in-home visits with clients to evaluate client well-being and to identify potential opportunities for positive impact.
- Work collaboratively with multidisciplinary Care Management team.
- Meet with clients, families, and other professionals to communicate findings and provide updates and recommendations.
- Provide supportive counseling and conflict resolution for clients, families, and outside providers to ensure well being of clients.
- Advocate for client needs with family and outside medical and non-medical professionals.
- Attend client medical appointments and provide transportation to appointments.
- Perform comprehensive in-home assessments and provide written and verbal summaries of findings and recommendations.
Knowledge & Skills Required
- Undergraduate and/or master’s degree in fields such as gerontology, social work, nursing, public health, health and human services, or other related fields.
- Minimum of two (2) years of experience working directly with older adults. Other relevant professional experiences may substitute.
- Self-starter and able to work independently.
- Superb organizational and time management skills.
- A passion for serving and working with older adults.
- Creative problem solver that can manage stressful situations.
- In-depth knowledge of the aging process and journey.
- Excellent verbal and written communication skills.
- Positive can-do attitude.
- Valid California driver’s license and insurance.

Pay: $65,000.00 - $75,000.00 per year
